Objective: To investigate the value of hemolytic disease of newborn(HDN)serological testing and related laboratory findings in HDN prediction,diagnosis and treatment monitoring,including ABO-HDN and rare and difficult RhD-HDN.Materials and Methods: Blood group serological examination was performed on 264 neonates diagnosed with ABO-HDN from May 2015 to June 2016 in the Affiliated Hospital of Qingdao University: blood type identification,hemolytic disease three tests(direct anti-erythrocyte resistance)Human globulin test,release test and serum free antibody test),irregular antibody,blood routine,reticulocyte percentage and bilirubin results were statistically analyzed,and clinical data and experiments of 4 pregnant and infants with RhD-HDN were performed.Room indicators were analyzed.Results: The 264 cases of neonatal detection of ABO-HDN were diagnosed.The results of three hemolysis tests: the highest percentage of the group was 67.2% positive for both the release test and the free antibody test,followed by the single positive test group for the release test 18.6%,the lowest proportion of the three groups were 11.2%,but the number of red blood cells and hemoglobin in the children with HDN positive in the three groups were significantly lower than those in the other groups.The percentage of reticulocytes was significantly higher than that in the other groups.The difference was statistically significant(p<0.05);4 cases of RhD-HDN,3 cases of hemolysis were positive,1 case of direct anti-test was positive,irregular antibodies were positive,and the results of irregular antibody identification were Rh.Anti-D2 cases,anti-E1 cases,anti-cE1 cases(ABO combined with Rh blood group system neonatal hemolytic disease),children with neonatal hemolysis symptoms within 24 hours.Conclusion: Prenatal maternal blood group identification and irregular antibody screening should be used as a routine obstetrics and gynecology routine for the prediction of HDN.Irregular antibody positive can be used as an effective basis for predicting hemolytic disease of Rh-HDN and ABO combined with Rh blood group system.Three tests of neonatal hemolysis,especially the loose test,are the most powerful serological tests for the diagnosis of HDN;the HDN neonates with positive positives in all three trials are severe;neonatal red blood cell count,hemoglobin,bilirubin test,and reticulocyte percentage can be As a laboratory-assisted dynamic monitoring indicator for clinical pediatric diagnosis and treatment of HDN.
