Objective: Meta-analysis was used to evaluate the clinical efficacy of perioperative outcomes and complications between intracorporeal and extracorporeal urinary diversion after laparoscopicradical cystectomy.provide evidence-based medical evidence for clinical surgical procedures.Thereby providing evidence-based medical basis for clinical surgical methods.Methods:Develop a search strategy,adopt a computer search model,and comprehensively search for electronic biological databases such as Pub Med,OVID LWW,Web of Science,VIP,CNKI,CMCI,and Wanfang database.Published domestic and international journals on clinical trials comparing the perioperative outcomes and complications between intracorporeal and extracorporeal urinary diversion after laparoscopicradical cystectomy.According to the relevant inclusion and exclusion criteria,the literature was selected and the literature information was extracted.According to the data type,the Rev-Man5.3 software provided by the Cochrane Collaboration Network was used for meta-analysis.The search time is from January 1,2000 to December 31,2018.The time required for the two surgical methods,the amount of bleeding during the operation,the length of hospital stay,and the total complication rate were evaluated.Results:This meta-analysis included a total of 6 English language literatures,involving a total of 455 patients.There are 240 people in the ECUD(Extra-Corporeal Urinary Diversion)group and 215 people in the ICUD(Intra-Corporeal Urinary Diversion group.The results showed that the amount of intraoperative blood loss in the ICUD group(WMD: 79.71,95% CI: 22.61 to 136.81,P<0.05)was less than that in the ECUD group;the total complications in the ICUD group after surgery(OR: 1.61,95% CI: 1.05)~2.46,P<0.05)is lower than the ECUD group.However,the ICUD group had two operation time(WMD:-25.05,95% CI:-74.78 to 24.68,P>0.05)and postoperative hospital stay(WMD: 1.46,95% CI:-0.63 to 3.54,P>0.05).There was no statistical difference between the aspects and the ECUD group.Conclusion: From the current study data analysis,laparoscopic radical cystectomy intracorporeal ileal bladder surgery is superior to laparoscopic radical cystectomy extracorporeal ileal bladder in the operation time and postoperative total complication rate;in terms of operation time and postoperative hospital stay Laparoscopic radical cystectomy is similar to extracorporeal ileal bladder surgery,but further clinical trial data from multiple centers and large samples are needed.
