Purposed: Little information about the relationship between lumbar lordosis and health-related quality of life(HRQOL)after lumbar surgery.This study aims to derive the lordosis Prediction equation using Pelvic incidence and establish a simple lumbar lordosis Prediction method to improve Postoperative quality of life.Materials and Methods: Sixty-two Patients with lumbar surgery Participated in the study.Spine-Pelvic Parameters and Oswestry Disability Index(ODI)were measured at the final follow-up.Identifying risk factors associated with good clinical outcomes through stepwise multiple regression model analysis using ODI.Oswestry Disability Index(ODI)was administered to evaluate HRQOL.The P patients with ODI under the 75 th percentile cutoff was designated into the “good” health related quality of life(HRQOL)group.In this group,P predictive equations of lumbar lordosis through sample regression analysis of the parameters and a reasonable LL angle was determined.Results: Forty-two patients with an ODI<29 Points(75th Percentile cutoff)were categorized into a good HRQOL group.The multiple regression analysis revealed lumbar lordosis(LL)(P <0.001),sacral slope(SS)(P = 0.003)and Thoracic kyphosis(TK)(P = 0.004)as the radiographic Parameters most significantly associated with ODI.The close relationship between incidence and other Pelvic and spinal parameters was obvious from the values of the regression coefficient: LL(r = 0.765,P <0.001),SS(r = 0.610,P <0.001),Pelvic tilting(PT)(r = 0.490,P <0.001),TK(r = 0.509,P <0.001).Lordosis was closely related to the orientation of the Pelvis,expressed by the SS(r = 0.669,P < 0.001)and TK(r = 0.676,P <0.001).Based on these correlations,the following predictive equations of the PI and lumbar lordosis were derived: LL = 0.59×PI+12(r = 0.765,P <0.001).Testing in the validation cohort revealed a good correlation between predicted LL and the actual LL(r = 0.522)with a mean absolute error(MAE)of 5.6°.Conclusions: This simple calculation method can provide a more appropriate and simpler prediction of lumbar lordosis for Chinese Population.It can be used as a decision-making tool for restoring LL in lumbar correction surgery and P lays an important role in improving the quality of life of patients after lumbar surgery. |
