Objective1.To investigate the effect of early comprehensive intervention on reducing the incidence of hypoglycemia and critical hypoglycemia in newborns with high risk of hypoglycemia, the recovery time of blood sugar value and the recovery effect.2.To explore the effect of early comprehensive intervention on the relationship between raising and maintaining neonatal body temperature and reducing the incidence of hypoglycemia.3.To explore the effect of early comprehensive intervention on reducing the risk of allergic diseases by adding formula milk.al transfer due to hypoglycemia.4.Through this study, the effective early comprehensive intervention measures for high-risk neonates with hypoglycemia were developed and applied in clinical practice.Method1.This research adopts randomized controlled research method, the selection in October 2018 to January 2019 in the province level of first-class hospital, maternal and newborn into the standard of 104 cases of the research object,52 cases were randomly divided into control group, experimental group 52 cases.2.Intervention methods:(1)Control group:blood glucose monitoring, postpartum routine nursing and neonatal routine nursing;Newborns with hypoglycemia and critical hypoglycemia were fed with formula milk and their blood glucose was monitored 1 hour after intervention.3.Evaluation index:blood glucose in neonatal peripheral blood, the incidence of neonatal hypoglycemia, the incidence of neonatal critical hypoglycemia, neonatal body temperature, and the time of neonatal blood glucose return to normal.(2)Experimental group:on the basis of routine care in the control group,3 hours after birth for continuous maternal and infant skin contact combined with biological rearing method for breastfeeding; Both hypoglycemia and critical hypoglycemia neonates in the control group received continuous maternal and infant skin contact combined with biological rearing for breast-feeding.Formula milk was added to the hypoglycemic newborn at the same time, and formula milk was not added to the critical hypoglycemic newborn when the blood glucose value was greater than 2.2mmol/L.3.Evaluation index:blood glucose in neonatal peripheral blood, the incidence of neonatal hypoglycemia, the incidence of neonatal critical hypoglycemia, neonatal body temperature, and the time of neonatal blood glucose return to normal.4.Statistical methods:all data were analyzed using SPSS22.0 statistical software.Result1.Fisher's exact probability method was used to compare the incidence of neonatal hypoglycemia between the two groups,P=0.028,less than 0.05,and the difference was statistically significant.Comparison of incidence of neonatal critical hypoglycemia X~2=0.036,P=0.85,greater than 0.05,with no statistical significance.2.Comparison of the peripheral blood trace blood glucose values before and after the intervention of hypoglycemia and critical hypoglycemia neonates in the two groups showed that the P values were all less than 0.05,and the difference was statistically significant. After intervention, the values of peripheral blood glucose and trace blood glucose in the two groups were all greater than 0.05,with no statistical significance.Comparison of the recovery time of hypoglycemia in newborns,T=17.500,P=0.002,less than 0.05,the differences were statistically significant.Comparison of critical hypoglycemia recovery time T=72.000,P=0.337,greater than 0.05,the difference was not statistically significant.3.The temperature of subjects in the two groups after intervention was compared,t=-21.558,P=0.000,less than 0.05,with statistically significant differences.4.Comparison of times of adding formula milk between the two groups(T=9.000,P=0.000,<0.05)showed statistically significant differences.Conclusion1.Early comprehensive intervention can effectively reduce the incidence of hypoglycemia in newborns with high risk without increasing the incidence of critical hypoglycemia. It can effectively increase the blood glucose of newborns with hypoglycemia and critical hypoglycemia, shorten the recovery time of neonatal hypoglycemia, and do not prolong the recovery time of critical hypoglycemia.2.Early comprehensive intervention has a significant effect on raising and maintaining neonatal body temperature, and the incidence of hypoglycemia decreases with the increase of body temperature.3.Early comprehensive intervention can effectively reduce the intake of formula milk and play a positive role in preventing allergic diseases.4.Early comprehensive intervention is safe, effective and feasible, which can be popularized and applied in clinical maternal and infant care.
