| Objective:To identify the relationship between the estimated fetal weight discordance and birth weight discordance,and evaluate the accuracy of predicting birth weight discordance of twin pregnancy.Methods:The application of ultrasound was used to detect the growth of twins at different gestation weeks,including biparietal diameter,femur length,humerus length,head circumference,anteroposterior trunk diameter,transverse trunk diameter,abdomen circumference which could calculate estimated fetal weight and analyze the data to compare the difference between fetal weight discordance and birth weight discordance.Results:About 15.9 percent of twins had birth weight discordance over 20 percent in our data(totally 277 cases)and monochorionic-diamniotic twin was more than dichorionicdiamniotic twin(17.5% and 15.2%,respectively).But there was no significant difference between the two groups(P>0.01).However,it was interesting that there was a significant difference in birth weight between twin pairs with and without severe ?BW(P<0.01)and the fetal weight was estimated by ultrasound last time was almost consistent of birth weight discordance.Furthermore,the negative predicted value of monochorionic-diamniotic twin was high when pregnancies were assessed by ultrasound at 22 ~ 24 weeks.Conclusion:It was very consistent that intertwin birth weight discordance was best predicted by an intertwin EFW discordance at the last ultrasound.Moreover,the birth weight of smaller fetus was the key factor to determine ?BW of twins.So we should pay more attention to EFW of the smaller fetus.If discordance was not suspected by ultrasound that EFW was more than 20%,especially of monochorionic-diamniotic twins at 22 ~ 24 weeks that EFW was less than 20%,the vast majority had no birth weight discordance and the s IUGR was almost not occurred in the future. |
