| Gastrointestinal malignancy is one of the most common malignancies in China.However,surgical resection of gastrointestinal malignancy is the most effective method for gastrointestinal malignancy,According to statistics,the overall complication rate of laparoscopic surgery for patients with gastrointestinal tumors is about 1.6-33.0%.Clinical studies have found that due to eating and absorption disorders and other reasons,surgical anesthesia can inhibit the immune function of patients with gastrointestinal tumors to a certain extent.Postoperative inflammatory reaction is relatively large,and postoperative recovery is not ideal,Therefore,improving the perioperative immune function of patients and reducing the incidence of postoperative complications have important clinical significance for postoperative recovery.This study is to evaluate the clinical value of perioperative application of thymopentapeptide in patients with gastrointestinal malignancy to improve immune function and short-term efficacy.Objective:In this study,60 cases of gastrointestinal malignant tumor surgery patients in our hospital were collected to investigate the changes of immune function of patients with perioperative application of thymus pentapeptide in gastrointestinal malignant tumor,The postoperative short-term efficacy(postoperative infection rate,incidence of anastomotic leakage,gastrointestinal function recovery time and hospital stay)was evaluated by analyzing the immune function,and the clinical application value was evaluated.Methods:Sixty patients who underwent laparoscopic radical gastrectomy for gastrointestinal malignancy in our hospital from January 2017 to October 2018 were randomly divided into two groups.30 cases in the observation group(routine treatment + thymus pentapeptide intramuscular injection of qd,1d before surgery to 7d after surgery),and 30 cases in the control group(routine treatment,including symptomatic support treatment of nutrition,anti-inflammatory,fluid rehydration,etc.).Postoperative immune function changes and recent clinical efficacy were compared between the two groups [complications: infection(incision infection,urinary tract infection,pneumonia),incidence of anastomotic leakage;gastrointestinal function recovery: exhaust time,defecation time;postoperative hospitalization days].Results:1 Immune function index contrast1.1 Postoperative 1 d compared with preoperative 1 d: Two groups of lymphocyte subgroup and immune 2 the value of the 1 d after surgery were significantly lower than the preoperative 1 d(P < 0.05).1.2 Postoperative 8 d compared with the postoperative 1 d: The indexes of observation group(CD3+,CD4+,CD8+,CD4+/CD8+)increased significantly on the 8th day after operation compared with the 1st day after operation(P < 0.05),while those of control group(except CD4+/CD8+)increased on the 8th day after operation(P < 0.05).However,the IgG value of the index observation group on the 8th day after operation was significantly higher than that of the 1st day after operation(P < 0.05),and the other indexes had no significant change(P > 0.05),while the indexes of the control group on the 8th day after operation had no significant change compared with the 1st day after operation in immune 2(P > 0.05).1.3 On the 8th day after operation,compared with the 1st day before operation: There was no significant difference in the lymphocyte subsets between the observation group(CD3+,CD4+,CD8+,CD4+/CD8+)and the control group(P > 0.05)on the 8th day after operation compared with the 1st day before operation,while the indexes in the control group on the 8th day after operation were still significantly lower than those on the 1st day before operation(P < 0.05).There was no significant difference in IgG,IgA,IgM and IgE on the 8th day after operation between the observation group and the 1st day before operation(P > 0.05),but the above indexes in the control group were significantly lower on the 8th day after operation than on the 1st day before operation in immune 2(P < 0.05).1.4 Comparison between the observation group and the control group: There was no significant difference in the indexes of lymphocyte subsets between the observation group and the control group on the 1st day before operation and the 1st day after operation(P > 0.05),while the indexes of the control group on the 8th day after operation(except CD4+/CD8+)were significantly lower than those of the treatment group(P < 0.05).There was no significant difference in IgG,IgA,IgM and IgE between the two groups on the 1st day before operation and the 1st day after operation(P > 0.05).On the 8th day after operation,the IgM value of the observation group was significantly higher than that of the control group(P < 0.05),but there was no significant difference in IgG,IgE and IgA between the two groups(P > 0.05).2 Comparison of postoperative recovery of gastrointestinal malignancyThe results showed that the infection rate in the observation group was lower than that in the control group(P < 0.05),and the length of hospital stay was shortened(P < 0.05),while the anastomotic leakage,exhaust time and defecation time were not significantly improved(P > 0.05).Conclusion:1 After radical resection,the stress of patients with gastrointestinal malignancy will also affect the immune function of the body to varying degrees after the surgical trauma,and further aggravate the immune suppression after the surgery.2 The application of thymus pentapeptide injection in patients with gastrointestinal malignancy during the perioperative period can significantly improve the cellular immune function(significantly increased lymphocyte subsets),and partially increase the humoral immune function.3 The application of thymopentapeptide in patients with gastrointestinal malignancy after surgery can reduce the incidence of complications such as wound infection,urinary tract infection and pneumonia,shorten the postoperative hospitalization days,and has statistical significance.There was no significant difference in the time of exhaust,defecation and anastomotic fistula.However,it can generally improve the postoperative recovery of gastrointestinal malignancy assisted by laparoscopy,reduce some hospitalization costs,and improve the short-term clinical efficacy of patients.It can be used in clinic. |
