Background & aim : exploring the factors related to the 1-year survival rate of patients with HBV-related hepatocellular carcinoma(HCC)treated with TACE,and providing a certain clinical basis for the short-term prognosis of TACE therapy in this population.Methods : According to certain inclusion exclusion criteria,a retrospective analysis of 151 patients with HBV-related liver cancer treated with TACE in our hospital from January 1,2017 to December 31,2017 was carried out.The last ALT,AST,ALB,TIBL,HBV-DNA quantity,HBs Ag quantity,HBe Ag quantity,AFP value,tumor diameter,number,portal vein invasion and liver cirrhosis before TACE were collected and sorted out.The time of first TACE was taken as the starting time,and the death time of the patient or until January 1,2019 as the end time.The collected data were analyzed and processed by SPSS18.0.Through univariate analysis,correlation analysis,survival curve drawing and multivariate analysis,the factors affecting the short-term outcome were gradually screened out.Result : Univariate analysis shows that HBs Ag quantification(P <0.001),AFP value(P < 0.001),tumor diameter(P < 0.001),tumor number(P ≤ 0.041)and portal vein invasion(P < 0.001)are significantly correlated with 1-year survival rate.There is a negative correlation between tumor diameter and HBSAg quantity(P < 0.001,correlation coefficient-0.319).Multivariate analysis finally screens out HBs Ag quantitative,tumor diameter,portal vein invasion,AFP quantitative four outcome factors.Discussion:Through univariate analysis,compared with the results obtained by multicenter study in affiliated Hospital of Sun Yat-sen University in recent years,we find that AST value is not related to the short-term survival rate of patients after TACE(P ≥ 0.230).The lower HBs Ag value correspond to the poor 1-year survival rate after TACE(P <0.001).Correlation analysis shows that the size of HBs Ag is negatively correlated with tumor diameter(P < 0.001,correlation coefficient-0.319),but the correlation is weak.The final COX model shows HBs Ag(P ≥0.044,HR ≥ 0.568)and AFP(P ≥ 0.044).HR=1.341),tumor diameter(P < 0.001,HR ≥ 1.936)and tumor portal vein invasion(P < 0.001,HR≥ 1.962)are correlated with 1-year survival rate.Conclusion : 1.The 1-year survival rate after TACE treatment of HBV-related hepatocellular carcinoma is correlated with HBs Ag quantity,AFP concentration,tumor diameter and portal vein invasion.2.There is a weak negative correlation between tumor diameter and HBs Ag quantitative size. |
