Objectives To study the change of white blood cell(WBC),neutrophil percentage(NEU %),red blood cell distribution width(RDW),platelet count(PLT),mean platelet volume(MPV),eosinophil count(EOS),interleukin-2(IL-2)and tumor necrosis factor-α(TNF-α)in patients with fungal infections secondary to chronic obstructive pulmonary disease,explore its relationship with the fungal infection.Methods 55 patients with fungal infections secondary to chronic obstructive pulmonary disease in respiratory medicine,the Affiliated of Hospital North China University of Science and Technology were selected as the cases of group,from November 2017 to November 2018.23 males and 32 females.60 patients with COPD without fungal infection admitted in the same week as the case group and with the same gender and age were selected as the control group.34 males and 26 females.Recorded and collected age,sex,body mass index,blood routine,and detected the levels of IL-2 and TNF-α in the all patients,compared the indicators between the cases of group and the control group,and analyzed its relationship with the fungal infection by calculating the OR value.Multivariate Logistic regression was used to screen the factors related to fungal infection.SPSS 22.0 was used for statistical analysis.Results 1 There were no significant difference in age,sex,body mass index between the control group and the cases of group(P>0.05).Compared with the control group,there was no significant difference in WBC and NEU% levels in the case group(P>0.05).2 The levels of IL-2,TNF-α,PLT,MPV,RDW,EOS of the cases of group were 249.38±51.19,40.88±12.10,141.31±59.22,9.71±1.13,12.58±1.74,0.66±0.12.The levels of IL-2,TNF-α,PLT,MPV,RDW,EOS of the control group were 227.74±24.13,34.65±9.70,175.47±58.87,9.19±1.19,12.12±1.62,0.59±0.13.The differences between the two groups were statistically significant.3 The mean of IL-2,TNF-α,PLT,MPV,RDW and EOS in the general population of the case group and the control group were respectively 238.09,37.63,159.14,9.44,12.26,0.62.Let IL-2,TNF-α,MPV,RDW and EOS be greater than or equal to the mean value for exposure,PLT be lower than he mean value for exposure,the six indicators were associated with COPD secondary fungal infection,and OR values were 6.706,3.250,5.764,3.273,4.083,4.421.The levels of IL-2,TNF-α,MPV,RDW and EOS increased,the levels of PLT decreased,and the risk of COPD secondary fungal infection increased.Multivariate Logistic regression analysis showed that IL-2 and PLT are associated with COPD secondary fungal infection(OR=6.076,95%CI: 2.168~17.028;OR=5.711,95%CI: 1.984~16.445).4 Among the patients with fungal infection,candida albicans is more common in aspergillus fumigatus and aspergillus flavus,and their composition ratio is respectively(51%,13%,5%,9%,2%).Conclusions 1 IL-2 and PLT are associated with secondary fungal infection of COPD.Elevated levels of IL-2,decreased levels of PLT,and increased risk of secondary fungal infections in COPD.2 Secondary fungus infection of COPD is given priority to with yeast and aspergillus.Figure0;Table20;Reference 152...
