Objective:To Retrospectively analyze the influencing factors of preterm birth in infertility patients after IVF-ET/ICSI treatment,in order to guide the development of individualized treatment and care programs to reduce the occurance of preterm birth after IVF/ICSI treatment in clinic.Methods:A total of 4005 couples who received IVF/ICSI treatment and had a live birth at a Reproductive & Genetic Hospital of Hunan Province between January 2016 to June 2017 were selected as the study subjects.They were divided into preterm birth group(631 couples)and full-term birth group(3374 couples)according to gestational age.Univariate analysis was used to compare the difference of the two groups of mothers,fathers,transplantation process and pregnancy related influencing factors between the two groups.Then the significant different factors(P<0.05)derived from univariate analysis were analyzed by multivariate unconditional logistic regression.The SPSS20.0 software was used for statistical analysis and the main statistical methods included descriptive statistics analysis,Mann-Whitney U test,Chi-square test,and unconditional logistic regression analysis etc.Results:1.Univariate analysis showed that there were 25 influencing factors for preterm birth(P<0.05),including:(1)Maternal factors before entering the treatment cycle(age,nationality,history of labor induction,history of ectopic pregnancy,serum concentrations of testosterone,albumin,triglycerides,and apolipoprotein B,activated partial thromboplastin time);(2)Paternal factors before entering the treatment cycle(BMI,systolic blood pressure,diastolic blood pressure,semen volume,total sperm motility,forward motor sperm rate,and normal morphology sperm rate);(3)Relevant factors in the process of transplantation(blastocyst transfer,sperm source,number of embryos transferred,whether transferred high quality embryos);(4)Influencing factors during pregnancy(multiple pregnancy,embryo reduction,gestational diabetes,hypertensive disorder complicating pregnancy,placenta previa).2.Multivariate analysis showed that 13 factors were independent influencing factors of preterm birth after IVF/ICSI:(1)Maternal factors before entering the treatment cycle(age,nationality,history of ectopic pregnancy,serum triglyceride concentration);(2)Paternal factors before entering the treatment cycle(systolic blood pressure,diastolic blood pressure,semen volum,total sperm viability);(3)Whether transferred high quality embryo;(4)Influencing factors during pregnancy(multiple pregnancy,embryo reduction,hypertensive disorder complicating pregnancy,placenta previa).Conclusions:Maternal factors before entering the treatment cycle(age,maternal ethnicity,maternal triglyceride concentration,history of ectopic pregnanc),paternal factors before entering the treatment cycle(systolic pressure,paternal diastolic pressure,semen volume,total sperm motility),whether transferred high-quality embryo,multiple pregnancy,embryo reduction,hypertensive disorder complicating pregnancy,placenta previa were independent influencing factors for preterm birth after IVF/ICSI. |
