| ObjectiveTo search the relationship between dermoscopic manifestations and pathological types of pigmented nevus(PN)and seborrheic keratosis(SK).MethodsA total of 143 patients(172 skin lesions)diagnosed as PN and 73 patients(82skin lesions)diagnosed as SK who came to our outpatient department from September 2018 to January 2019 were selected.Patients diagnosed by dermoscopy and histopathological examination were divided into different groups according to their dermoscopic and topathological features,we need to analyze the corresponding relationship between dermatoscopic manifestations and pathological types.SPSS 19.0software was used to process datas.Results1.General condition of patients with different pathological types of PNOf the 143 patients with PN,42 were male and 101 were female.Male to female ratio was 1:2.40.The age ranged from 12-81 years old.The average age was36.11±13.78 years old.Among 172 cases of PN,they were classified according to histopathological features:92 were IDN,62 were CN and 18 were JN.IDN and CN were most common in the head and neck;JN was most common in the palmar area.2.Dermoscopic examination results of PNOf the 172 skin lesions with PN,59 were globular pattern,45 were homogenous pattern,24 were reticular pattern,5 were peripheral globules with central network or homogenous and the starburst pattern,2 were peripheral reticular with central globules,13 were two component pattern,16 were multicomponent pattern,4 were parallel furrow pattern,2 were fibrillary pattern,2 were lattice-like pattern.3.The corresponding relationship between dermatoscopic manifestations and histopathological types of PNIn the dermascopic pattern of the three types of PN,there were many globular pattern,reticular pattern and homogenous pattern.Comparison of the proportion of three pigment patterns in three types of pigmented nevus,the difference wasstatistically significant(P=0.018).The common dermoscopic patterns of IDN were the globular pattern and the homogenous pattern.The most common dermoscopic pattern of CN was the globular pattern,and the most common pattern of the JN was the reticular pattern.Vascular structure found in the skin rash of 39 cases,34 were IDN,5 were CN.Curved blood vessels were most common in IDN.CN has more regular linear vessels and curved blood vessels.4.General condition of patients with SKOf the 73 patients with SK,24 were males,49 were females,male to female ratio was 1:2.04.The age ranged from 31-86 years old.The average age was 60.79±11.93 years old.SK was most common in the trunk,head and neck.5.Histopathological diagnosis results of SKOf the 82 skin lesions with SK,36 were acanthotic SK,25 were hyperkeratotic SK,20 were adenoid SK,1 were irritated SK,0 were clonal SK.6.Dermoscopic examination results of SKOf the 82 skin lesions with SK,19 were the type of plaque,17 were the type of verrucous hyperplasis,28 were the type of flat papules,18 were the type of velvet-like.7.The relationship between dermoscopic and histopathological types of SKThe main histopathological types of the four types of SK under dermatoscopy were different,the difference was statistically significant(P<0.05).The most common pathological type of plaque type SK is hyperkeratotic SK,and followed by acanthotic SK;The most common pathological type of verrucous hyperplasia type SK is adenoid SK;The most common pathological type of flat papular SK is acanthotic SK;The most common pathological type of velvet-like SK is hyperkeratotic SK,and followed by acanthotic SK and adenoid SKConclusionThere is a certain correspondence between dermoscopic manifestations and pathological types of PN and SK.Dermoscopy is a good assistant tool for clinical diagnosis. |
