Objective:Compare the expression level of serum galactose lectin-3(Galectin-3)in patients with Acute coronary syndrome(ACS)complicated with heart failure and in different cardiac function grades,then explore the clinical significance of Galectin-3 expression in patients with ACS complicated with heart failure,in order to evaluate the severity of cardiac function.Methods:In this study,61 patients with ACS complicated with heart failure were included from the Department of Cardiology,Affiliated Hospital of Yan'an University from September,2017 to December,2018.The diagnostic criteria of ACS referred to the 8th edition of the internal science text book of the People's Medical Publishing House.The diagnostic criteria of heart failure was based on the 2014 Chinese Heart Failure Diagnosis and Treatment Guidelines.At the same time,the control groups were randomly selected in our hospital,including 50 patients diagnosed as ACS and 50 healthy people who were checked by physical examination.According to the clinical characteristics,patients with ACS complicated with heart failure were further divided into two groups,one group was unstable angina(UA)complicated with heart failure(n=35),which included cardiac function of grade?(n=12),grade?(n=14)and grade?(n=9);the other group was acute myocardial infarction(AMI)complicated with heart failure(n=26)which included cardiac function of grad?(n=12),grade?(n=14)and grade?(n=9)respectively.All subjects were examined by 12-lead ECG(18-lead ECG were performed if needed)immediately after they were admitted to hospital.If necessary,multiple reexaminations were performed to monitor the dynamic evolution of ECG.The Pro-BNP and general clinical data of all subjects were collected.And Pro-BNP was detected by enzyme-linked immunosorbent assay(ELISA)in the laboratory of our hospital.At the same time,placed the blood samples of all subjects in the clotting-promoting tube for 30 minutes and the serum was separatedafter centrifugation.Then marked and sealed it,and placed it in-80? refrigerator cryopreservation.Finally,using ELISA detected serum Galectin-3 together.The level of serum Galectin-3 and Pro-BNP as well as general and clinical data were compared among control group,ACS group and groups of ACS complicated with heart failure group.The levels of serum Galectin-3 and Pro-BNP were compared in two subgroups of ACS(including UA group and AMI group).And the levels of serum Galectin-3 and Pro-BNP were compared in two subgroups of ACS complicated with heart failure(including UA complicated with heart failure and AMI complicated with heart failure).The levels of serum Galectin-3 and Pro-BNP were compared in the UA complicated heart failure group and AMI complicated with heart failure group of cardiac function grade II,grade III and grade IV.The correlation of serum Galectin-3 and c Tn I?Pro-BNP were detected by Pearson correlation analysis.The correlation of serum Galectin-3 and the grade of cardiac function in NYHA and Killip were detected by Spearman grade correlation analysis.The experimental data were analyzed by SPSS20.0 analytic software package.Result:1.Comparing general and clinical data among control group,ACS group and ACS complicated heart failure group,there were differences in c Tn I,serum Galectin-3 and Pro-BNP(P<0.05).And there were no difference in other indexes(P>0.05).2.There were difference in serum Galectin-3?Pro-BNP between the groups of UA and AMI(P<0.05).3.Serum Galectin-3?Pro-BNP were different between UA complicated with heart failure and AMI complicated with heart failure(P<0.05).4.Serum Galectin-3 and Pro-BNP were different among UA complicated with heart failure groups including complicated with NYHA grade II,grade III,and grade IV(P<0.01),there were also significant differences between each other(P < 0.01),and the expression level of serum Galectin-3 and Pro-BNP in patients with grade ?of cardiac function was higher than that in patients with grade II and grade III.5.Serum Galectin-3 and Pro-BNP were different among AMI complicated with heart failure groups including complicated with Killip grade II,grade III,and grade IV(P<0.01),there were also significant differences between each other(P < 0.01),and the expression level of serum Galectin-3 and Pro-BNP in patients with grade ?of cardiac function was higher than that in patients with grade II and grade III.6.Serum Galectin-3 has positive correlations respectively with all of them included c Tn I,Pro-BNP,and cardiac function grade of NYHA and Killip.Conclusion:1.The expression of serum Galectin-3 was higher than that in patients with ACS.2.The expression of serum Galectin-3 in patients with ACS complicated with heart failure was higher than that in patients with simple ACS.3.Serum Galectin-3 can help to assess the severity of heart function impairment in patients with ACS complicated with heart failure.
