| Objective:1.The characteristics of measurement of lumbar pedicle parameters by MSCT MPR technique were analyzed.2.To explore whether there are left-right and gender differences in lumbar pedicle related parameters,for the surgical treatment of lumbar vertebrae related diseases,provide anatomical parameters before operation,guide pedicle screw placement during operation,improve the accuracy and safety of pedicle screw placement.Methods:From October 2016 to November 2018,140 patients(70 males and 70 females)who needed to receive lumbar spiral CT scan in the orthopaedic outpatient department or inpatient department of Yan ’an University affiliated hospital were randomly selected for this study.Using the UTC-760 128-row spiral CT machine produced by produced by Shanghai united imaging technology co.,ltd.The patient was placed in the supine position,and the L1~L5 spine was continuously scanned.The obtained original images were transferred to the lenovo post-processing software workstation,and the images(layer thickness 0.625 mm,window width 2600 HU,window position 800 HU,bone window)were obtained by using CT multi-plane reconstruction technology(MPR).The measurement indexes included height of the narrowest point of the lumbar pedicle(OPH),width of the narrowest point of the lumbar pedicle(OPW),height of the narrowest point of cancellous bone(CBH),width of the narrowest point of cancellous bone(CBW),length of pedicle axis(PAL),and angle of transection of pedicle(TPA)with a precision of 0.01 mm.SPSS 23.0 software is used for statistics,analysis and processing of the obtained data.The results were expressed as mean± standard deviation(`x±s).The comparison between groups was performed by independent sample t test.The test level was α=0.05,p<0.05.The difference was statistically significant.Results:1.Observe the shape of the coronal plane at the narrowest point of the lumbar pedicle: L1 presents longitudinal oval or kidney shape(92%),irregular shape(8%);L2 was in longitudinal oval or kidney shape(94%),irregular shape(6%).L3 is of round or kidney shape(95%)and irregular shape(5%).L4 is of spindle or teardrop shape(93%)and irregular shape(7%).The L5 is laterally oblong(89%)and irregularly shaped(11%).2.In the same gender,all the measurement indexes of the left and right sides of the pedicle of the same vertebral body showed no statistical significance(p > 0.05);OPH,OPW and PAL of the same vertebral body were significantly different between different genders(p < 0.01).In the same vertebral body,there was no significant difference of TPA between the male group and the female group(p > 0.05).Conclusions:MSCT MPR technology can accurately measure the relevant parameters of the lumbar pedicle,visually display the local anatomy of the lumbar pedicle,and observe and analyze the relevant parameters of the measurement.In the surgical treatment of lumbar vertebra related diseases,MSCT MPR technology has the following values: providing anatomical parameters preoperatively,guiding the pedicle screw placement intraoperatively,and improving the accuracy and safety of pedicle screw placement.In the same gender,all the measurement indexes on the left and right sides of the pedicle of the same vertebral body are similar;OPH,OPW and PAL in the same vertebra were significantly greater in males than in females.;TPA in the same vertebra both male and female.was similar. |
