Characteristics Of Syndrome Elements Of Syndrome Drift In Syndrome Of Congestion Of Dampness Turbidity In Non-alcoholic Fatty Liver Disease

Objective: To recognize the syndrome drift in syndrome of congestion of dampness turbidity in non-alcoholic fatty liver disease(NAFLD),and analyze the characteristics of syndrome element of syndrome drift in NAFLD with congestion of dampness turbidity,to learn the laws of syndrome changes in the course of the disease process,to consummate the theory of dynamics in traditional Chinese medicine,and to provide basis for improvement of differentiation of syndromes and treatment for NAFLD.Aiming at exploring the dynamic course of disease development and providing evidence for diseases prevention and treatment.Methods:Ninety-seven cases of NAFLD patients with congestion of dampness turbidity syndrome,treated in inpatient department from September 2017 to September 2018 in Xiamen Hospital of Traditional Chinese Medicine,were enrolled in this study.Patients were followed up for up to three months.During the scheduled 3-month follow-up period,it would be ended after the occurrence of syndrome drift.Researcher would keep detailed notes of patient’s information on TCM four consultations once per fortnight,and the emerging of symptom drift would be assessed by using the Traditional Chinese Medicine Health Identification System(developed by Fujian University of Traditional Chinese Medicine Syndrome Study Base).Then the syndrome elements of patients in the syndrome drift group,including disease location and nature syndrome elements,were collected and the syndrome elements characteristics were summarized and analyzed.Results:(1)Syndrome drift occurred 39 in 97 patients(40.2%)in this study.(2)There was no statistical difference in the distribution of gender,age,AST,TBil,TG,TC,LDL-c,FPG in baseline data between syndrome drift and non-syndrome drift groups,there was significant statistical difference in the distribution of BMI(F=8.726,P=0.004),ALT(F=4.541,P=0.038),GGT(Z=-2.627,P=0.009),NAS scores(Z=-4.001,P<0.001).The baseline BMI,ALT,GGT,NAS scores of the syndrome drift group were higher.(3)Statistics analysis of syndrome elements which the score ≥70 in syndrome drift group,show disease location syndrome elements were existed between pre-and post syndrome drift include spleen,liver,exterior,heart spirit,heart,lung,gallbladder,semi-exterior-interior and kidney.The emerging disease location syndrome was large intestine.The nature of disease elements were existed between pre-and post syndrome drift include Qi deficiency,humidity,yang deficiency,qi stagnation,phlegm,heat,yin deficiency,summer heat,blood stasis,ingestion and fluid retention.The emerging nature of disease elements were blood deficiency,yang-hyperactivity,cold and body fluid deficiency.(4)Statistics analysis of all syndrome elements in syndrome drift group,disease location syndrome elements which existed between pre-and post syndrome drift were liver and spleen.Kidney and semi-exterior-interior were the emerging disease location syndrome elements when syndrome drift occurred.The disease nature syndrome elements were existed between pre-and post syndrome drift include phlegm,yang deficiency,qi deficiency,qi stagnation and humidity.Emerging nature syndrome elements when syndrome drift occurred were blood stasis.Conclusion:(1)Syndrome drift was found frequently in syndrome of congestion of dampness turbidity in NAFLD and 40.2% patients emerged syndrome drift.Moreover,condition of the patients with syndrome drift were more likely to become serious,result from that syndrome drift was more commonly in patients with high BMI,high levels of ALT,GGT and high NAS scores.(2)Patients with high BMI,high levels of ALT,AST and high NAS scores are liable to have syndrome drift and most of the patients with syndrome drift have more serious illness.In the syndrome of congestion of dampness turbidity in NAFLD,the disease location syndrome elements liver and spleen were visible throughout the course.The new disease location syndrome elements were kidney and semi-exterior-interior;And the nature of disease syndrome elements,that throughout the course,were phlegm,yang deficiency,qi deficiency,qi stagnation and humidity,while blood stasis became the new syndrome elements for nature of disease in syndrome drift.
