Effect Of Bushen Huoxue Granule On The Postoperative Efficacy Of Ilizarov Tibial Transverse Transportation For Wagner Stage 2-4 Diabetic Foot

ObjectiveTo study the effect of Bushen Huoxue Granule on diabetic foot(DF)treated by transverse tibial transport,and to observe whether it has more advantages than simple hypoglycemic and anti-infective conventional post-operative treatment,and whether it can improve the clinical symptoms and quality of life of patients earlier.MethodSixty-eight diabetic foot patients who met the diagnostic criteria and inclusion criteria of the study were divided into two groups by random number table method: 1.30 cases in the observation group were treated with routine hypoglycemic and anti-infective treatment after transverse tibial transplantation;2.30 cases in the control group were treated with Bushen Huoxue Granule on the basis of routine treatment after transverse tibial transplantation.There were 47 males and 15 females.There were 24 left feet and 38 right feet.Wagner grade 2 was 14 cases,Wagner grade 3 was 37 cases,Wagner grade 4 was 11 cases.All patients received active comprehensive medical treatment after admission.Ilizarov tibial transverse bone transfer was performed in our orthopedics and traumatology department after signing the consent letter.Seven patients with ulcer and severe infection underwent vacuum sealing drainage(VSD)before operation,and 12 patients underwent necrotic toe extubation at the same time.The transverse tibial movement began on the 5th day after operation(1 mm/day,3 times).After 21 days,the tibial fragments were removed in the opposite direction for 21 days and fixed for 4 weeks.After 12 weeks,all the indexes were followed up and evaluated.Observation Indicators: Visual Analogue Score VAS,skin temperature,ankle brachial index ABI,Toronto clinical scoring system TCSS score and lower limb CTA.Results1.There was no significant difference between the two groups in sex,age,course of disease,Wagner stage and various observation indexes before treatment(P > 0.05),which was of comparative research value.2.After 12 weeks of treatment,VAS score,skin temperature,ankle-brachial index,TCSS score and lower limb CTA were compared between the two groups.VAS score: Compared with the control group,the treatment group decreased significantly(P < 0.05);compared with before treatment,both the observation group and the treatment group decreased significantly(P < 0.05).Skin temperature: Compared with the control group,the treatment group decreased significantly(P < 0.05);compared with before treatment,both the observation group and the treatment group decreased significantly(P < 0.05).ABI: Compared with the control group,the treatment group decreased significantly(P < 0.05);compared with before treatment,both the observation group and the treatment group decreased significantly(P < 0.05).TCSS score: Compared with the control group,the treatment group decreased significantly(P < 0.05);compared with before treatment,both the observation group and the treatment group decreased significantly(P < 0.05).Blood flow velocity of posterior tibial artery: Compared with the control group,the reatment group decreased significantly(P < 0.05);compared with before treatment,both the observation group and the treatment group decreased significantly(P < 0.05).Conclusion1.Ilizarov transverse tibial transporting therapy can achieve good clinical efficacy for patients with Wagner stage 2-4 diabetic foot.2.For Wagner stage 2-4 diabetic foot patients undergoing transverse tibial transfer,Bushen Huoxue Granule has more advantages than simple hypoglycemic and anti-infective routine post-operative treatment: it can improve the clinical symptoms of patients earlier,improve the quality of life,and may accelerate the process of microcirculation reconstruction to a certain extent.It is worth popularizing and applying in clinic.
