The Effects Of Modified Wuteng Zhiwang Decoction Combined With Celecoxib Capsule For Ankylosing Spondylitis Belonging To Damp-heat Obstruction Type

Objective: To observe the clinical efficacy and safety of modified wuteng zhiwang decoction combined with celecoxib capsule in the treatment of ankylosing spondylitis of damp-heat obstruction type,so as to provide theoretical basis for the clinical application of modified Wuteng zhiwang decoction.To seek the better scheme of treating ankylosing spondylitis with combination of traditional Chinese and western medicine.Methods: A randomized controlled study was used in this study.According to diagnosis,inclusion and exclusion criteria,a total of 80 patients diagnosed with AS belonging to damp-heat obstruction type were collected,who visited the Department of Orthopedics,Xiamen Hospital,Beijing University of traditional Chinese Medicine from January 2018 to December 2018.They were randomly divided into two groups: control group(n=40)and experimental group(n=40).The control group was given celecoxib capsule orally,and the experimental group was treated with modified Wuteng zhiwang decoction on the basis of the control group.The two groups were taken continuously for 12 weeks,and the patients were educated about the disease.Correct living habits education and guidance function exercise.The observation records were compared between the two groups after treatment for 6 weeks and 12 weeks.Bath ankylosing spondylitis disease activity index(BASDAI)score,Bath Anakylosing Spondylitis Functional Index(BASFI)score,Thoracic activity,Schober test,occipital wall distance,C-reactive Protein(CRP),erythrocyte sedimentation rate(ESR)and other indexes and adverse reactions.The data obtained were analyzed and analyzed by SPSS 20.0.Results: 1.During our treatment,75 patients completed the clinical study.Two patients were detached from the experimental group(38 cases in total),and three were detached from the control group(37 cases in total),which were not significant difference between two groups(P>0.05).2.Before treating,statistical analysis of gender,age,course of the disease,BASDAI score,BASFI score,Thoracic activity,Schober test,occipital wall distance,CRP,ESR were performed,which were not significant difference between two groups(P>0.05).3.After treatment for 6 weeks and 12 weeks,there were obviously improved at BASDAI score,BASFI score,CRP,ESR etc.in both two groups.After 12 weeks of treatment,there were improved at Thoracic activity,Schober test,occipital wall distance etc.compared to before(P<0.05).Comparing with two groups’ data,it was statistical significance that the improvements of BASDAI score,BASFI score,Thoracic activity,Schober test,occipital wall distance,CRP,ESR etc.of the experimental group during 12 weeks’ treatment were better than that of the control group.However,during 6 weeks’ treatment,it was only improved these indicators,which were BASDAI score and CRP,that the experimental group was better than the control one(P<0.05).4.After 12 weeks of treatment,the number of cases in the control group meeting the ASAS20 standard was 18,and the effective rate was 48.65%,while the number of cases in the experimental group meeting the ASAS20 standard was 29,and the effective rate was 76.32%.The clinical efficacy of the experimental group was better than that of the control group.There was a statistical difference(P<0.05).5.The incidence of adverse reactions in the experimental group was lower than that in the control group,and no significant adverse events occurred in both groups during the treatment.Conclusion: The total curative effect of modified wuteng zhiwang decoction combined with celecoxib capsule in treating ankylosing spondylitis of damp-heat obstruction type is better than that of celecoxib capsule alone,and the adverse reaction is not increased and the safety is good.It provides a basis for the further clinical application of the program.It is suggested that the modified Wuteng Zhiwang decoction is effective and safe in the treatment of damp-heat obstruction type AS.
