Summary Of Professor Xikui Zhang’s Clinical Experience In Treating Chronic Renal Failure

ObjectiveThe characteristics of the deportation and medication of chronic renal failure were summarized,which providing more reference and new ideas for the effective treatment of chronic renal failure.MethodsProfessor Xikui Zhang’s clinical experience was collected CRF cases from September 2013 to December 2018 in the Department of State Clinic of Fujian University of Traditional Chinese Medicine and The Second People’s Hospital of Fujian Province and set up a database of Professor Zhang’s treatment of CRF cases.SPSS 23.0 and SPSS Modeler 18 software was used to analyze the regularity of CRF treatment for Professor Zhang by frequency analysis,association rules,and cluster analysis.Results100 cases involved 126 drugs.Different herbs appeared a total of 3017 times,with the most frequent 14 herbs being:Astragalus,Rhubarb,Monkshood,Poria,Ginseng,Millet sprout,Malt,Eucommia bark,Alisma,Membrane of chicken gizzard,Oyster,Parasitic loranthus,Herba Dendrobii,Tangerine peel.And there are 17 kinds of medicines for treating this disease.The top 12 medicines used most are tonic drugs,damp-draining diuretic,digsstive medicine,purgative medicinal,and interiorwarming drug,heat-clearing medicine liver-pacifying and wind-extingushing drug,qi-regulating drug,diaphoretic,antirheumatic,astringent,bloodactivating and stasis-resolving drug.The main drug pairs are rhubarb andmonkshood,rhubarb and astragalus membranaceus,rice sprout and malt,poria and rhizoma alismatis,barrewwort and curculigo rhizome,fructusligustri lucidi and eclipta,lily bulbandtulipbulb,radix bupleuri and radixscutellariae,cortexdictamni and belvedere fruit,etc.The coreparties are as follows.1.CRF basic side:rhubarb,monkshood,astragalus membranaceus,ginseng,eucommia bark,teasel root,rice sprout,malt,ingluvin,medicated leaven,fructus amomis.2.Jueyinshaoyangjubing:radix bupleuri,radixscutellariae,the tuber of pinellia,dangshen,cassiatwig,poria,bighead atractylodes rhizome,rhizoma alismatis,betelnut peel,semen plantagcuis,motherwort,ramulus taxilli,purple perilla,pericarpium citri reticulatae,immature bitter orange,fruit of hawthorn.3.Obvious deficiency of yang:Curculigo rhizom,barrewwort,agrimonia,radixmorindae,seed of chinese dodde,gorgon fruit,cherokee rose.4.Deficiencyof liver-yin and kidney-yin:angelica sinensis,radixpaeoniae alba,rhizoma gastrodiae,gambirplant,oyster,turtle shell,fructusligustri lucidi,eclipta,sealwort,dendrobe,seed of wild jujube.5.Detoxification and turbidity relief and dispel the wind and invigorate the circulation ofblood: lily bulb,tulip bulb,greenbrier,pyrrosia lingua,coix seed,cortexdictamni,belvedere fruit,cicada shell,papaya,yellow grass,root of kudzu vine.Conclusions1.Professor Zhang believes that chronic renal failure belong to Jueyinshaoyang disease,based on the syndrome-differentiation of the six meridians,the characteristics of CRF are oasthenia in origin and asthenia in superficiality,cold-heat in complexity.And spleenand kidney decline is the essence.Water,damp heat,blood stasis,turbidity,and windevil are important pathogenic factors.2.First of all,at the time of diagnosis and treatment,Professor Zhang pays attention to the three integrated treatments of disease differentiation,dialectics and syndrome differentiation.Second,he thinks that spleen-kidney yang deficiency is more common in chronic renal failure.So he uses rhubarb aconitesoup as the basic recipe.Then he adjustes the drug according to changes in qi and yang.Such as obvious deficiency of yang was supplemented by Sanxian Decoction,and deficiency of liver-yin and kidney-yin was supplemented by Erzhi Pills,and blood deficiency was supplemented by Danggui BuxueDecoction,Qi deficiency and collateralsstasis was supplemted by Buqi Tongluo recipe,Jueyin Shaoyang disease was supplemted by Xiaochaihu Decoction and Wuling Powder.At last,he according to the patient’s current discomfort,modificates prescription with syndrome.3.The characteristics of medication:Hegives prominence to strengthening healthy qi.Besides,he psys attention to clear dampand promote diuresis.And activating blood circulation and removing blood stasis through the whole process and that he is careful with the drugs of breaking blood and expelling blood stasis.Among that he attachs importance to cleare away heat and toxic materials.
