Objective Application of combining traditional Chinese and western medicine treatment in childre n with Primary nephrotic Syndrome(Primary Nephritic Syndrome,PNS).Observing th e changes of urine Aquaporin-1,Aquaporin-2 and Aquaporin-7,in order to further impr ove the pathogenesis of pediatric PNS and elucidate the mechanism of combined tradit ional Chinese and western medicine in the treatment of pediatric PNS((pure type),so as to improve the clinical efficac. Methods Select 20 cases, the People's hospital affiliated to fujian university of traditional Chine se medicine and the second affiliated People's hospital outpatient and hospitalization in accordance with PNS(pure type)in the diagnosis of children as treatment group. And 20 healthy children were selected as the control group in the physical examination ce nter of our hospital.By combining traditional Chinese and western medicine(Shenkan gling),observing the treatment group before and after the treatment of urinary AQP1,AQP2 and AQP7 level change and serum albumin, cholesterol, and 24 hours urinary protein quantitative change.SPSS 25.0 statistical software was used for statistical analys is of the data. Results 1.There was no significant difference between the treatment group and the control gro up in gender and age composition(P all >0.05)before treatment. And the levels of uri nary AQP1,urinary AQP2 and urinary AQP7 in the treatment group were significantl y higher than those in the control group(P all <0.05).2.After 4 weeks of treatment, the levels of urine AQP1 and urine AQP2 in the treatme nt group were significantly lower than those before treatment(P all<0.01)while there was no statistical significance in the levels of urine AQP7(P>0.05);Compared with t he healthy group, the levels of urine AQP1 were significantly higher than the healthy group(P<0.05),while there was no statistical significance in the levels of urine A QP2 and AQP7(P all>0.05).24-hour urinary protein quantification, serum cholesterol levels were significantly lower than before treatment(P all<0.01),serum albumin lev els were significantly higher than before treatment(P<0.01).3.After 12 weeks of treatment, the levels of urine AQP1 and urine AQP2 were signifi cantly decreased compared with those before treatment(P all < 0.01),while there was no statistical significance in the levels of urine AQP7(P>0.05);compared with those after 4 weeks of treatment, there were no statistical significance in the levels of urine AQP1,AQP2 and AQP7(P all>0.05);compared with the healthy group, there was no statistical significance in the levels of urine AQP1,AQP2 and AQP7(P all>0.05).The24-hour urine protein quantitation and serum cholesterol level in the treatment group were significantly lower than those before treatment(P<0.01),and serum albumin leve l was significantly higher than those before treatment(P<0.01).4.After 12 weeks of treatment,18 patients with PNS had complete remission and 2 p atients with partial remission. Conclusions 1.The levels of urine AQP1,urine AQP2 and urine AQP7 in pediatric PNS were sig nificantly increased, suggesting that they may be involved in the pathogenesis of pedia tric PNS.This may be related to the distribution of AQPs on renal tubules and their involvement in renal resorption.AQPs has the function of selective filtration in spatial structure and is the molecular basis for maintaining the balance of human water and liquid metabolism.2.The application of integrated Chinese and western medicine in the treatment of ped iatric PNS(pure type)can significantly reduce the levels of AQP1 and AQP2 in urine, suggesting that the combined Chinese and western medicine in the treatment of pedia tric PNS(pure type)may achieve the treatment goal by improving the renal tubular f unction and reducing the levels of AQP1 and AQP2 in urine.3.The combined treatment of traditional Chinese and western medicine has significant curative effect in reducing abnormal elevated urinary protein, blood lipids and elevate d serum albumin, suggesting that kidneys-invigorating and blood-activating therapy can improve the disorder of lipid metabolism and hypercoagulability in children,so as to alleviate the condition of children and improve the clinical efficacy. There were 18 p atients with complete clinical remission and 2 patients with partial remission.
