The Correlation Between Hyaluronan Synthase 2 And Diabetic Nephorpathy In Serum

Objective To discuss the relationship between the early diabetic nephorpathy(DN)and the level of hyaluronan synthase-2(HAS-2)in peopel who suffers from the diabetes mellitus,find out a sensitively indicator to diagnosis the diabetic nephorpathy.Methods 90 cases of T2 DM patients in Endocrine Department of GanSu People’s Hospital were selected,including 32 cases without complication(DM group)and 29 cases of early diabetic nephropathy(DN group).Another 29 healthy examiners were selected as the control group(NC group).Measured the level of HAS-2,urinary protein,urinary albumin excretion rete(UAER),urine microalbumin(mAlb),creatinine(Cr)and endogenous creatinine clearance rate(Ccr)and glycosylated hemoglobinAlc(HbA1c),fasting blood glucose(FBG),interleukin-6(IL-6)、toll like receptor 4(TLR4)、c-reactive protein(CRP)in serum.And note on the some general data.Results The level of HAS-2 for DN group is higher than the other groups(P<0.05),meanwhile,the result of DM group is significantly higher than the NC group(P<0.05).The concentration for DN group is significantly higher than the DM group of urinary protein、mAlb、UAER、IL-6、CRP、TLR4(P<0.05).The result of HOMA-IR、Ccr、HbA1c、FBG for NC group is significantly different with other groups(P<0.05).The year of disease in the DN group was last more year than that in the other groups(P<0.05),and the DM group’s year of disease was longer than the NC group(P<0.05).The time of disease has the positively relationship with the concentration of HAS-2 mAlb and UAER 、 urinary protein、HbA1c、FBG、IL-6、HOMA-IR、TLR4(P<0.05).Conclusion(1)The concentration of HAS-2 in serum for early DN patients wassignificantly higher than that in those who did not develop to renal injury due to diabetes and those who were healthy,indicating that HAS-2 was obviously correlated with the state of an illness.(2)The concentration of HAS-2 in serum for early DN patients was preferentially changed than urine protein、 mAlb、Ccr 、UAER and so on,which could be used as a sensitive early auxiliary diagnostic index of DN.(3)The concentration of HAS-2 in serum is obviously correlated with the course of illness.The level of HAS-2 is closely related to the degree of kidney injury in diabetes mellitus,and the longer the age is,the kidney disease is more severe.(4)The longer the course of disease and the worse the blood glucose were controlled in T2 DM patients,the more significantly change of HAS-2 was.(5)If other inflammatory factors like IL-6、 CRP and TLR4 is decreased while HAS-2 is increased,simultaneous observation of these items can be used as the observation indicators of disease progression,so as to remind the doctor to intervention for such patients as soon as possible.
