| Objectives 1.Through monitoring the changes of various physiological indicators to evaluate the stress response of nursing students during high-pressure operation so as to improve the related research, and provide data support for psychological stress intensity assessment.2.To explore the relationship between personality traits and psychological stress intensity, and further discover which personality traits have an advantage in coping with stress and stable operation. It provides meaningful reference for on-the-job screening of nurses’ stress-related work and the selection for disaster relief, and helps nursing students to understand their own personality traits to more effectively match the appropriate work direction, and correctly carry out career planning.3.To provide guidance for the training of departments with high stress intensity in working and personnel training before rescue operations,to help nursing staff to better adapt to high-pressure and high-intensity work, and lay the foundation for follow-up intervention training plan.4.To help understand the stress physiological response of works with high stress intensity, such as pilots,to provide scientific methods and evidence for the analysis of special personnel positions and personnel selection. Methods 1.Clinical qualitative interviews and transcription of interview data.To understand the main pressure sources of nurses and determine the related factors about the later monitoring test, such as the type of operation of the research objects and the type of pressure sources.2.Real-time monitoring and scene simulation. We organized the research objects to do relevant operations in different pressure source scenes. In the process of it,factors such as environment and procedure are strictly controlled. The basic physiological indicators, heart rate, skin temperature and skin resistance of operator are captured by wearing relevant instruments. The heart rate variability is analyzed by time domain analysis method to obtain the indicators of time domain analysis. Finally, statistical analysis was carried out on the collected basic physiological indicators and the heart rate variability indicators(the second part used one-way analysis of variance and the third part used repeated measures analysis of variance).3.Questionnaire survey.Firstly,we used 16 PF to test the personality traits of the subjects by DXC-6 multi-series psychological assessment instrument. At the end of the monitoring test, the subjects were grouped according to the standard score of 10 personality traits that may be related to stress state. Then analyze the differences of indicators between low, middle and high groups. Results 1.Modeling Analysis of Basic Physiological Indicators and Heart Rate Variability in Monitoring Test 1.1 Analysis results of basic physiological indicators Heart rate and skin temperature had significant difference in resting period, baseline period and stress period(F=88.25,P<0.05;F=42.91,P<0.05),and the value was basically as follows: stress period > baseline period > resting period. There was no significant difference between the two indicators in resting period and baseline period(P>0.05),but there was significant difference between baseline period and stress period(P<0.05).Heart rate had statistical difference under time-limited pressure and assessment pressure(P<0.05),but had no statistical difference under other pressure conditions(P>0.05);skin temperature had no statistical difference under three pressure conditions(P>0.05).The skin resistance fluctuated greatly, and there were significant differences in resting period, baseline period and stress period(F=1897.08,P<0.05).The basic manifestations were resting period > baseline period > stress period. There was no significant difference between resting period and baseline period(P>0.05).There were significant differences between baseline period and stress period(P<0.05).There were also significant differences in skin resistance under three stress conditions(P<0.05).1.2 Analysis of Heart Rate Variability The six indexes of heart rate variability had significant difference in resting period, baseline period and stress period(P<0.05).Except Mean HR,the values of other indexes were basically as follows: stress period > baseline period > resting period.1.2.1 Mean HR and Mean RR Mean HR and Mean RR values had no significant difference between resting and baseline periods(P>0.05),but there were significant differences between baseline and stress periods(P<0.05).There was statistical difference between time-limited stress and assessment pressure(P<0.05),and no statistical difference between time-limited stress and task interference pressure(P>0.05).At the same time, there was no statistical difference between assessment pressure and task interference pressure(P>0.05).1.2.2 SDNN and RMSSD There was no significant difference between SDNN and RMSSD at rest and baseline(P>0.05),but there was significant difference between baseline and stress(P<0.05).Under the three stress conditions of SDNN,only the difference between assessment pressure and time-limited pressure had statistical significance(P<0.05),but there was no statistical difference between other stresses(P>0.05).RMSSD had significant difference under three stress conditions during the stress period, and the difference between time-limited pressure and other two stress conditions was statistically significant(P<0.05).There was no significant difference between assessment pressure and task interference pressure(P>0.05).1.2.3 NN50 and PNN50 NN50 and PNN50 also showed a downward trend with the increase of stress intensity, which was consistent with the changes of SDNN and RMSSD.There was no significant difference in the values of NN50 and PNN50 between resting and baseline periods(P>0.05).There was significant difference in the values of NN50 and PNN50 between baseline and stress periods(P<0.05).There were statistical differences among the three stress conditions during the stress period(P<0.05).2.Analysis of the correlation between personality traits and physiological indicators of stress 2.1 Analysis of Stability Personality Traits and Physiological Indicators The results of Pillai’s trace of heart rate, skin temperature and skin resistance all showed that the P value of the interaction of stress level * grouping was less than 0.05,which indicated that the interaction of stress level * grouping contributed greatly to the stress personality model. There were significant differences in the interaction between stress level and grouping of the three basic indicators(P<0.05).The "stress level * grouping" interaction P value of Pillai’s trace results for all indicators of heart rate variability was less than 0.05.Mean HR,Mean RR,SDNN,RMSSD,NN50 and PNN50 had significant differences in the interaction between "stress level" and "grouping"(F=27.60,22.62,18.66,40.28,29.21,P<0.05).2.2 Analysis of Excitatory Personality Traits and Physiological Indicators The results of Pillai’s trace of heart rate, skin temperature and skin resistance showed that the interaction P value of stress level * grouping was less than 0.05.There were significant differences in the interaction between stress level and grouping of the three basic indicators(P<0.05).The "stress level * grouping" interaction P value of Pillai’s trace results for all indicators of heart rate variability was less than 0.05.Mean HR,Mean RR,SDNN,RMSSD,NN50 and PNN50 had significant differences in the interaction between "stress level" and "grouping"(F=6.90,5.33,5.00,8.01,9.34,P<0.05).2.3 Analysis of Tension Personality Traits and Physiological Indicators Pillai’s trace results of heart rate, skin resistance and skin temperature all showed that the P value of the interaction of stress level * grouping was less than 0.05,which indicated that this effect contributed greatly to the model. There were significant differences in the interaction between stress level and grouping of heart rate and skin temperature(P<0.05),but there was no significant difference in the interaction between stress level of skin resistance and grouping(P>0.05).The "stress level * grouping" interaction P value of all indicators of heart rate variability was less than 0.05.Mean HR,Mean RR,SDNN,RMSSD,NN50 and PNN50 had significant differences in the interaction between "stress level" and "grouping"(F=5.17,3.49,18.66,6.01,4.74,P<0.05).2.4 Analysis of Adaptation and Anxiety Personality Traits and Physiological Indicators The results of Pillai’s trace of heart rate, skin temperature and skin resistance showed that the P value of the interaction of stress level * grouping was less than 0.05,which indicated that the interaction of stress level * grouping contributed greatly to the model. There were significant differences in the interaction between stress level and grouping of the three basic indicators(P<0.05).The "stress level * grouping" interaction P value of other indicators of heart rate variability except SDNN value was less than 0.05.Mean HR,Mean RR,SDNN,RMSSD,NN50 and PNN50 had significant differences in the interaction between "stress level" and "grouping"(F=4.38,4.40,3.22,5.95,4.35,P<0.05).2.5 Analysis results of emotional and safe alert personality traits and physiological indicators The results of Pillai’s trace of skin temperature and skin resistance showed that the interaction P value of stress level * grouping was less than 0.05,and the corresponding value of heart rate was more than 0.05.There were significant differences in the interaction between stress level and grouping of the three basic indicators(P<0.05).The "stress level * grouping" interaction P value of Pillai’s track results of other indicators of heart rate variability except NN50 was less than 0.05.Mean HR,Mean RR,SDNN,RMSSD,NN50 and PNN50 had significant differences in the interaction between "stress level" and "grouping"(F= 4.71,4.43,5.00,4.67,5.02,P< 0.05).The other five personality traits are sensitivity,anxiety,self-discipline,introversion and extroversion, cowardice and courage. There was no statistical difference in the results of basic physiological indicators and heart rate variability analysis(P>0.05).The results showed that there was no correlation between them and the changes of stress physiological indicators of the subjects. Conclusions 1.This study effectively established the stress operation model of nursing students under different stress conditions.Through the analysis of the indexes in the three periods of the model, the disturbance factor of the change of physiological index value caused by the operation action itself in the model could be neglected. At the same time, the results of the test model show that those pressure intensities can stimulate the operator’s stress state.2.The change trend of physiological indexes and heart rate variability is basically consistent with the degree of tension. With the increase of tension, the individual manifested with the increase of heart rate, the appropriate increase of skin temperature, the decrease of skin resistance and the decrease of time-domain analysis index of heart rate variability, reflecting the weakening of autonomic nervous regulation function, the decrease of parasympathetic nervous activity in the balanced regulation of sympatheticparasympathetic nervous system, the increase of sympathetic nervous activity and its predominance in stress state.3.People with different personality traits have different physchological stress intensities under stress.Stability, excitability, tension and the secondary personality factors including adaptation and anxiety, emotionality and alertness, are correlated with psychological stress intensity to some extent.4.The people with emotional stability, calmness, adaptability and rational alertness are more advantageous in rescue or first aid work.Their physiological indicators fluctuate less and regulation ability is stronger in stress scenes.Nursing students who are easily emotional, impulsive and impatient are relatively less advantageous in high stress work. Therefore,it is very necessary to add personality traits to rescue personnal selection and career planning.5.This study shows that physiological evaluation indicators have important significance in the evaluation of stress intensity. If combined with the indicators of psychological evaluation, the stress state can be more comprehensively evaluated and studied. In the next study of the stress state of military workers, the two aspects should be fully combined to evaluate the stress response more systematically and scientifically. |
