The Analgesic Efficacy Of Fascia Iliaca Compartment Block Vs.Caudal Block After Hip Orthopedics In Children: A Comparative Study

Objective To compare the postoperative analgesic effects of fascia iliaca compartment block（FICB）and caudal block in children who are undergoing orthopedic surgery for development dysplsia of the hip.Methods 62 paediatric patients undergoing orthopedic surgery for development dysplsia of the hip under general anaesthesia were recruited to the study and randomly divided into two groups by using SPSS:caudal block group（group C,n=31）;fascia iliaca compartment block group（group F,n=31）.Assessing the effect and safety of FICB on some outcomes,including perioperative vital signs,pain scores,sedation scores,the use number of dezocine and ibuprofen,sunstance P,adverse side effects of24 hours after the extubation and the time of first urination after operation.Results Compared with group C,the FLACC,CRIES,and Oucher pain scores decreased in group F at T₈（P<0.05）,and the Riker sedation-agitation scores decreased in group F at T₇ and T₈（P<0.05）.The first postoperative urination time of group F was shorter than that of group C（P<0.05）.Compared with group C,MAP,HR,PAED score of group F at T₃,T₄ and T₅ increased（P<0.01）,the incidence of postoperative nausea and vomiting in group F increased（P<0.05）,and the use of dezocine in group F increased significantly at 24h after surgery（P<0.01）.There was no significant difference in serum substance P concentration between the two groups at each time point after surgery（P>0.05）.Conclusions Ultrasound-guidance fascia iliaca compartment block has longer analgesia time and few adverse reactions related to nerve block,which can be safely used for perioperative analgesia of pediatric development dysplsia of the hip;However,due to the wide range of caudal block,the effect of fascia iliaca compartment block in early postoperative analgesia is not better than that of caudal block.
