The Diagnostic Value Of Ultrasound In Hashimoto’s Thyroiditis And Thyroid Nodule

Objiective 1.To investigate the ultrasound(US)imaging features of the malignant thyroid nodules with or without Hashimoto’s thyroiditis background,and evaluate its diagnostic value.2.To evaluate the ultrasound imaging characteristics of Hashimoto’s thyroiditis with the presence of benign and malignant thyroid nodules,and study the diagnostic value of it.Methods 1.This retrospective analysis included 309 cases of malignant thyroid nodules confirmed by pathology in our hospital.Among these cases mentioned above,156 thyroid nodules with Hashimoto’s thyroiditis and 153 cases without Hashimoto’s thyroiditis were divided into two groups in comparison.The preoperative US imaging characteristics of nodules were investigated,including: number of thyroid nodules,location of the nodules,size of the nodules,maximum diameter of nodules,aspect ratio,morphology and the types,boundary,internal structure,echo type,homogeneity of internal echo,halo sign,calcification and the types,posterior echo attenuation,the degree of blood supply and the vascular pattern of the hyroid nodules,the level of thyroid capsule contact area and elastography scores etc.2.This retrospective analysis included 213 cases of Hashimoto’s thyroiditis confirmed by pathology and biopsy results,in which including 140 cases with malignant thyroid nodules and 73 cases with the presence of benign thyroid nodules.Divided into two groups,the preoperative ultrasound imaging characteristics of the thyroid glands were evaluated as follows: the size of the thyroid glands,echo type and the blood supply of the thyroid glands.Results 1.In the background of Hashimoto’s thyroiditis malignant thyroid nodules had significant differences in the location of the lesion,morphology and the types,boundary,halo sign,homogeneity of internal echo,calcification and the types,posterior echo attenuation etc.(P < 0.05).2.In the cases of Hashimoto’s thyroiditis with benign and malignant thyroid nodules,the size of the thyroid glands and blood supply had statistical difference(P < 0.05).Conclusions In the ultrasonography diagnosis,malignant thyroid nodules with Hashimoto’s thyroiditis background have certain specificity.Meanwhile,Hashimoto’s thyroiditis with malignant thyroid nodules also has certain characteristics.Thus we can also make further efforts in this aspect of research.In a word,this study is meaningful in the clinical ultrasound diagnosis of thyroid nodules in the Hashimoto’s thyroiditis background patients.
