Objective: To analyze the medium-term and long-term efficacy of Viatorr stent-graft in treating patients with portal hypertension by transjugular intrahepatic portosystemic shunt(TIPS),and to evaluate the feasibility,safety and clinical effect of Viatorr stentgraft.Methods: 44 patients with portal hypertension,who hospitalized in the Department of Interventional Radiology of the First Affiliated Hospital of China Medical University from October 2015 to November 2018,were treated by TIPS with Viatorr stent-graft.Results: Intrahepatic portovenous shunt was successfully established in all 44 cases,and the technical success rate was 100%.A total of 44 Viatorr stents were implanted with a diameter of 8 mm.The portal vein pressure was significantly decreased from(32.87 ±6.05)mmHg to(22.47 ±6.37)mmHg(P < 0.05).The portal pressure gradient(PPG)was also significantly decreased from(26.88 ±6.12)mmHg and(11.52 ±4.65)mmHg(P < 0.05).There was no significant change in blood ammonia at 1 day after operation compared with that before operation(P > 0.05).The total bilirubin increased,prothrombin time prolonged,albumin decreased and INR increased 1 day after operation.There were significant differences in the four indicators(P< 0.05).The median follow-up period was 14 months.The total mortality rate was 13.64%(6/44): three of them died of end-stage liver disease with multiple organ failure,one died of upper gastrointestinal bleeding,one died of cerebral hemorrhage and one died of pelvic tumors.The 1-year,2-year and 3-year survival rates were 93.10%,78.78% and 56.27%,respectively.The post-operation stent patency rate of 1 year,2 years,3 years was 89.66%,75.86% and 65.02%.The incidence of hepatic encephalopathy was 18.18%(8/44).The rebleeding rate was 9.10%(4/44).Conclusion: The Viatorr stent has high technical success rate in TIPS for patients with portal hypertension.There are high stent patency rate and low incidence of hepatic encephalopathy.
