| Purpose:Observe the clinical symptoms of patients with T2 DM combined with SCH and patients with T2 DM without combined with SCH,compare and analyze the relevant indicators,and obtain the traditional Chinese medicine constitution type and related factors of T2 DM combined with SCH,and select RCT literatures of Chinese herbal prescriptions combined with L-T4 to cure SCH in nearly 20 years in order to provide evidence-based medical evidence for the treatment of SCH.Material and method:Paper I: From the electronic medical records system of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine,randomly select 45 patients with T2 DM combined with SCH as the observation group,and another randomly select 45 patients with T2 DM without combined with SCH were selected as control group,compared observation group and control group age,gender,disease duration,BMI,waist circumference(cm),systolic blood pressure,diastolic blood pressure(mm Hg)fasting blood glucose,glycated hemoglobin,FT3,FT4,TSH,Tg Ab,Tpo Ab,TG,CHOL,HDL-C,LDL-C,Apo A1,Apo B,and record the common complications of T2 DM between the two groups,such as:CVD,AS,DR,DKD DPN.Analysing the traditional Chinese medicine constitions according to the symptoms when they were entering the hospital,using SPSS 21.0 software to analyze the statistics.Paper 2: Using "subclinical hypothyroidism","subclinical hypothyroidism","SCH","traditional Chinese medicine treatment","warmness spleen and kidney","invigorating the spleen and kidney" as keywords to search CNKI,million Fang Database,Wei Pu,reviewed the clinical RCT study of subclinical hypothyroidism treated with traditional Chinese medicine compound combined with L-T4 in the past 20 years.Meta-analysis was performed on the included literatures with Rev.Mans5.3.Results:Paper One:(1)The prevalence of female patients who have T2 DM combined with SCH is higher than that of male patients in the same condition,and it is closely with the emotional and physical functions differently between male patients and female patients.(2)There were significant differences in BMI,WC,hypertension,blood lipids,and blood glucose between the observation group and the control group(P<0.05),suggesting that SCH can aggravate the severity of the above indexes in T2 DM patients.(3)The incidence of CVD and AS in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of DR,DKD and DPN between the two groups(P>0.05).(4)There was no single traditional Chinese medicine constitions type in the two groups,and combined with 2 or 3 constitions were common.The constitution type of the observation group was qi deficiency,yang deficiency and blood stasis,and the constitution type of T2 DM in the control group was qi deficiency and blood stasis.Quality> Yang deficiency,and the number of qi deficiency and yang deficiency in the observation group and the control group were statistically different(P<0.05).Paper 2: The meta-analysis results showed in the conditions ofthe observation group was treated with L-T4 combined with Chinese herbal prescriptions.The control group was only treated with L-T4.The TSH,FT4,LDL-C,TC,total effective rates in the experimental group and the control group are significant difference(P < 0.05).Conclusion:(1)There is a significant difference between the observation group and the control group in gender,MS-related indicators,and CVD complications of DM in the study 1(P < 0.05),which may be related to abnormal TSH in patients with T2 DM combined with SCH.(2)The distributive ratio of TCM constitution in the observation group and the control group in Paper 1are differently.There are significant differences between the two groups in the number of qi deficiency and yang-deficiency physique(P<0.05).(3)The doctor can diagnosis and curing the disease according to the condition of traditional Chinese medicine constitions,the DM patients need to periodic monitoring the thyroid functions.(4)Chinese medicine combined with L-T4 replacement therapy can effectively relieve TSH,FT4,LDL-C,TC in SCH patients. |
