Strategies For Improving The Hierarchical Diagnosis And Treatment Of Chronic Diseases In H Community Health Service Center

With the development of the economy,the medical level has also shown leaps and bounds.People pay more and more attention to medical health-care,and the requirements for health gradually switches from treatment to prevention.In addition to the high standards of medical quality,patients pay more attention to medical services and quality of life.At the same time,China’s chronic patient population is becoming bigger and bigger.In recent years,it has become an upward trend year by year.Faced with such a large group,medical resources are in a shortage situation,especially for the shortage of tertiary medical resources.The state has formulated a policy of grading diagnosis and treatment for this purpose,hoping to achieve the purpose of “quick and divided,up and down linkage”,but the tension and distrust of the doctor-patient relationship has caused a large number of people to be reluctant to grade down and become obsessed with the third-level qualification.Infatuation with the authority of experts has led to an increase in the imbalance of medical resources.In this paper,the H Community Health Service Center is used as a pilot,and the chronic disease grading diagnosis and treatment is the main research subject.The H community health service center collected and analyzed relevant medical data from the organizational structure,basic operations,drugs use and testing of chronic diseases.Through the questionnaires and satisfaction questionnaires for patients with chronic diseases and medical staff,we can find out the specific problems of current graded diagnosis and treatment and the causes of these problems.In order to improve the purpose of graded diagnosis and treatment,this thesis has developed a family doctor contract service strategy for chronic disease patients in the H Community Health Service Center.The purpose of this service is “patient-centered” family-based medical care,through the “combination of prevention and treatment”.And it is also allowes chronically ill patients to receive comprehensive,sustainable,long-term professional care.From the aspects of business process design,benefit distribution mechanism design,patient participation promotion strategy and technical support of informationization for the family doctor contract service,the paper designs and elaborates in detail,and hopes to use the method of family doctor contract service.The family doctor contract service can improve the basic medical service model,give full play to the advantages of “general practitioners” at the primary level,improve the quality of primary medical services,and enhance the trust of patients in primary care,fundamentally solve the problem of medical resource allocation,and achieve a truly effective goal of graded diagnosis and treatment.At the end of the article,through the implementation of the family doctor contract service,the relevant data were collected about medical effects,patient satisfaction,medical staff satisfaction,community patient flow,community economic benefits,etc.,and passed to the family doctor.The comparison of data before and after the signing service clarifies the effect of family doctors’ contracting services on the classification and treatment of chronic diseases and provides reference for other primary medical institutions to effectively realize the classification and treatment of chronic diseases.
