Objective: To acquire the prevalence,incidence and treatment gap of epilepsy in some rural areas of Hainan Province,To provide reference for the prevention and treatment of epilepsy in rural areas of Hainan Province.Methods: In this study,residents of Chengmai County,Danzhou City,Baoting Li-Miao nationality Autonomous County and Dingan County in Hainan Province were investigated by household.The investigation was divided into two stages,In the first stage,By the unified training of neurology resident,graduate students and clinical major senior students in a group of 2 people,to fill in the form of “Epidemiological investigation of Epilepsy preliminary screening form”.We used a screening questionnaire for epilepsy developed by the World Health Organization(WHO,2000),The questionnaire consists of 13 questions.focus on the analysis of theprevalence,the incidence of epilepsy,genderdifferences and ethnicdifferences in the prevalence,age distribution,seizure types,treatment.In the second stage,The selected epilepsy patients and suspected epilepsy patients were reviewed by the chief physician of neurology who had clinical experience,and the patients were interviewed again,and the history of the confirmed cases was inquired into in detail,and the registration form of the confirmed epilepsy cases was re-filled out.Results: We surveyed 16,676 subjects and identified 51 PWE.the prevalence of epilepsy was 3.06 per 1,000.the standardized prevalence was 4.87 per 1,000.the mean annual incidence was 23.99 per 100,000.The total prevalence of active epilepsy was2.39 per 1,000.The prevalence of epilepsy in the Han nationality was 3.27 per 1,000.The prevalence of epilepsy in Li nationality was 2.27 per 1,000.There was no significant difference in epilepsy prevalence between Li nationality and Han nationality by chi-square test.The prevalence of male and female was not significantdifference by chi-square test.The prevalence of epilepsy was highest at the 50-59 and more than 60 year age groups.The first onset age of epileptic patients was between0-9 year age groups,followed by 10-19 year age groups.Among the patients with epilepsy,there were 15 cases of symptomatic epilepsy,accounting for 29.41 per 100 of the total number of epilepsy,among which,stroke was the most common cause of symptomatic epilepsy.The most common category was generalized seizures,accounting for 68.63 per 100 of total epilepsy.the active epilepsy treatment gap in this study was therefore 58.97%.Conclusions:(1)The prevalence and incidence of epilepsy in some rural areas of Hainan Province were close to the previous investigation level in other parts of China.the types of epileptic seizures are mainly generalized seizures.(2)The prevalence of epilepsy in some rural areas of Hainan province showed two peaks,20 to 29 years old and 60 years after age.(3)there was no significant difference of nationality and sex in epilepsy.(4)The treatment gap of active epilepsy in some rural areas of Hainan Province is relatively large(58.97%).
