Objective:To observe the clinical effect of acupotomy overall release surgery combined with stellate ganglion block(SGB)in the treatment of cervical vertigo(CV),explore a better clinical solution for the treatment of cervical vertigo..Methods:90 patients with cervical vertigo who met the conditions in February 2018 to December2018 were selected by a simple randomized grouping method,divided into Groups A,B and C,30 cases in each group.The patients in the group A were treated with acupotomy overall release surgery:1st "T"-shaped needle knife overall release surgery;2nd time to release the attachment points and tenderness points of the atlantoaxial soft tissue,once a week,alternating use,4 weeks for a course of treatment.Group B were treated with stellate ganglion block by ultrasound,twice a week,bilateral swaps,4 weeks for a course of treatment.Group C used acupotomy overall release combined with stellate ganglion block,and stellate ganglion block was performed 2nd and 4th days after acupotomy release surgery.The clinical symptoms and functional table scores of cervical vertigo were performed before treatment,after treatment,and 3 months after treatment.Before and after treatment,Three groups of patients were examined by transcranial Doppler(TCD),to observe the changes of the mean flow velocity(Vm)、end diastolic velocity(Vd)、peak systolic velocity(Vs)、resistance index(RI).Finally,the overall efficacy of each group was analyzed.All data were analyzed by the software of SPSS20.0.Results:1.The first three groups of patients in the treatment of gender,age,course of disease,ESCV,TCD and other aspects of situation is basically consitent,no statistical diferences(P>0.05),with the significance of comparison.2.Total effective analysis: After treatment,the total effective rate of group C was96.7%,group A was 83.3%,and group B was 73.3%.The efficacy of the three groups was statistically treated,the difference between the three groups was statistically significant(P<0.01).Group C can improve the clinical treatment efficiency of patients with cervical vertigo.3.ESCV:The three groups had significant difference in the improving “vertigo”,“neck and shoulder pain”,“headache” and “daily life work”,and the combined group was superior to the single treatment group(P<0.01).However,there was no statistical difference among the three groups in the psychological and social adaptation scores(P>0.05).After 3 months of treatment,compared with the post-treatment,the combined group and acupotomy group had significant difference in improving “vertigo”,“neck and shoulder pain”,“headache” and“daily life work”,and the combined group was superior to the acupotomy group(P<0.01).4.TCD: The results of Vm、Vs、Vd and RI in the three groups of vertebral-basal arteries before and after treatment showed significant difference(P<0.01),and the combination group was superior to the single treatment group(P<0.01).Conclusion:1.Acupotomy overall release surgery combined with stellate ganglion block can betterimprove the clinical symptoms of patients with cervical vertigo,and effectively improve the quality of daily life of patients.2.Acupotomy overall release surgery combined with stellate ganglion block can also improve the average blood flow velocity of vertebral-basal artery and reduce vascular resistance,improve clinical The curative effect provides a better treatment for the clinical treatment of cervical vertigo.
