| Objective:To evaluate the efficacy and safety of zhen gan san yu decoction by observing the changes of hematoma volume,NIHSS score and TCM syndrome scores before and after treatment of patients with acute cerebral hemorrhage,Extreme Hyperactivity Liver-yang and Wind-fire Invading Upward Syndrome. Methods:A randomized controlled clinical study was used.A total of 70 patients who met the inclusion criteria were enrolled. They were randomly divided into observation group and control group(35 cases each). Both groups were guided by the guidelines for the treatment of cerebral hemorrhage developed by the Chinese Medical Association’s 2014 Guidelines for the Diagnosis and Treatment of Cerebral Hemorrhage, and were routinely treated according to the clinical path of acute cerebral hemorrhage developed by the Department of Encephalopathy of our hospital. After treatment with Zhengan Sanyu Decoction, the two groups were treated for 14 days. The hematoma volume,NIHSS score,TCM syndrome score on the 7th day and the 14 th day of treatment were observed. The coagulation function, liver and kidney function, and electrocardiogram were observed as the safety indicators. Finally, the relevant data were statistically analyzed and evaluated, and the efficacy and safety of "zhen gan san yu decoction" in treating patients with acute cerebral hemorrhage, extreme hyperactivity liver-yang and wind-fire invading upward syndrome were evaluated.Results:(1)This clinical study included 70 patients,65 patients with clinical observation,2 patients in the observation group,3 patients in the control group, and the total shedding rate was 7.14%.(2)Before the treatment, there were no significant differences in the general conditions between the two groups in terms of gender, age, blood pressure, cerebral hemorrhage and associated symptoms(P>0.05).(3)Results of curative effect analysis:The total effective rate of the observation group after treatment(87.87%)was significantly better than that of the control group(71.87%),and there was significant difference between the two groups(P=0.043<0.05).(4)Volume of hematoma:On the 7th day of treatment, the volume of hematoma in both groups was significantly lower than that before treatment(P<0.01).The degree of hematoma volume reduction in the observation group was better than that in the control group, and there was a significant difference between the two groups(P=0.009<0.01).On the 14 th day of treatment, the volume of hematoma in both groups was significantly lower than that before treatment(P<0.01).The degree of hematoma volume reduction in the observation group was also better than that in the control group, and the difference was significant between the two groups(P=0.001<0.01).(5)NIHSS scores:On the 7th day of treatment, the NIHSS scores of the two groups were significantly lower than those before treatment(P<0.01).The reduction of NIHSS score in the observation group was better than that in the control group, and there was a significant difference between the two groups(P=0.031<0.05).After 14 days of treatment, the NIHSS scores of the two groups were significantly lower than those before treatment(P<0.01).The reduction of NIHSS score in the observation group was also better than that in the control group. The difference between the two groups was significant(P=0.035<0.05).(6)TCM syndrome scores:On the 7th day of treatment, the TCM syndrome scores of the two groups were significantly lower than before treatment(P<0.01).The reduction of TCM syndrome scores in the observation group was better than that in the control group, and there was a statistical difference between the two groups(P=0.001<0.01).On the 14 th day of treatment, the scores of TCM syndromes in the two groups were significantly lower than those before treatment(P<0.01).The reduction of TCM syndrome scores in the observation group was also better than that in the control group, and there was significant difference between the two groups(P<0.01).(7)Safety analysis:There was no clinical significance in the changes of liver and kidney function, coagulation function and electrocardiogram before and after treatment. There was no significant difference between the two groups(P>0.05).Conclusion:(1)Zhen gan san yu decoction combined with conventional Western medicine treatment can promote acute cerebral hemorrhage, extreme hyperactivity liver-yang and wind-fire invading upward syndrome patients with hematoma volume reduction, improve neurological deficit symptoms,in the body, dizziness, headache, face red The symptoms of idiopathic irritability, dry mouth, bitterness, redness, redness, constipation, etc.have been improved.(2)Zhen gan san yu decoction did not find obvious adverse reactions during the treatment, and the patients had good safety by coagulation function, liver and kidney function, and no abnormal electrocardiogram. |
