| Objective:To collect the data of primary biliary cholangitis(PBC),summarize the clinical features,calculate the survival curve,explore the prognostic risk factors affecting PBC,and provide data for clinical work.Methods:Used retrospective cohort study,collected the data of patients with Primary biliary cholangitis who were hospitalized for the first time from July 2008 to July 2015 in Ningxia Medical University General Hospital were.Observed and recorded general data,medical imageology data,and endoscopic findings.Survival rate was calculated by life table method,Kaplan-Meier method was used to analyze the prognostic factors.Statistical variables were introduced into multivariate Cox regression model analysis.P <0.05 for the difference was statistically significant.Results:1.Clinical features:218 patients with an average age of 58.24±11.47 years of age,84.5%of female,15.6%of male.55%(120/218)patients with typical symptoms,manifested as fatigue,abdominal discomfort,skin itching and so on.PBC patients often associated with autoimmune diseases(21.6%)and infectious diseases(20.6%).2.Survival analysis:The average survival time of patients with PBC was from 0.02 years(73days)to 7.77 years.The 1-year survival rate was 89%;3-year survival rate was 70%;5-year survival rate was 58%.3.PBC patients dieaverage age(61.69±10.96)years old.The main causes of death were gastrointestinal bleeding(36.9%),infectious diseases(32.2%).4.Kaplan-Meiermethodof 29 PBC prognosis affect the single-factor results(p<0.05).These are Age,Gender,Red blood cell count,PT,Albumin,blood sodium blood calcium,ALBI、Child-pugh and so on.5.The significant single factor analysis results were introduced into COX multivariate analysis model,Suggesting that age,gastrointestinal bleeding,ascites,serum sodium levels,Calcium levels,prothrombin time,IgA,ALBI,Child-Pughare are the main factors that affect the prognosis of primary biliary cholangitis.(p<0.05)Conclusions:1.PBC more common in middle-aged women.Early symptoms lack specificity,often combined with other autoimmune diseases.2.In this study,PBC 5-year survival rate was 58%.The main cause of death is upper gastrointestinal bleeding.3.Age,Repeated gastrointestinal bleeding,hyponatremia,hypocalcemia,IgA,ALBI grade,Child-Pugh classification is the main factor affecting the prognosis of PBC. |
