Objective: To investigate the correlation between urine albumin to creatinine ratio(UACR)and the severity of coronary heart disease(CHD)in elderly patients with hypertension.Methods: 126 patients with suspected CHD from Jnly 2017 to October 2018,aged 65 to 83 years,were enrolled in the study.All subjects had indications for coronary angiography and underwent coronary angiography,and tested UACR during hospitalization.The subjects were divided into normal coronary artery group(43 cases)and coronary heart disease group(83 cases)according to the results of coronary angiography.General clinical data,coronary angiography results and UACR were collected.We compared general clinical data and UACR between the coronary heart disease group and the normal coronary artery group.According to the results of coronary angiography,the SYNTAX scores of each patient in the coronary heart disease group were calculated,and then divided into three groups according to the results of SYNTAX scores: 44 cases in the low risk group(< 22 points),27 cases in the middle risk group(23 points-32 points)and 12 cases in the high risk group(> 33 points).The differences of general clinical data and UACR among the three subgroups were compared.Then the correlation between UACR and severity of coronary artery disease was analyzed.Result: 1.Comparison of general data and UACR between CHD group and normal coronary artery groupThere were no statistical difference in age,sex,diabetes history,smoking history,hypertension course and grading,ACEI/ARB drug use,body mass index(BMI),systolic blood pressure(SBP),diastolic blood pressure(DBP),fasting blood glucose(Glu),cholesterol(TC),triglyceride(TG),low density lipoprotein(LDL-C),Serum creatinine(Scr)between CHD group and normal coronary artery group(P > 0.05).However,there was significant difference in high density lipoprotein(HDL-C)and UACR between the two groups(P < 0.05).2.Analysis of the correlation between UACR and coronary heart disease in elderly patients with hypertensionIn elderly patients with hypertension,UACR was a risk factor for coronary heart disease(OR= 1.01,95%CI:1.00—1.02,P< 0.05);HDL-C was a protective factor for coronary heart disease(OR= 0.14,95%CI:0.01—0.43,P< 0.05.3.Comparison of general data and UACR among three subgroups of CHD groupThere were significant differences in UACR and HDL-C among the three groups(P < 0.05).Otherwise there was no significant difference in age,sex,diabetes history,smoking history,course and grading of hypertension,ACEI/ARB drug use,BMI? SBP? DBP? Glu? TC?TG? LDL-C?Scr(P > 0.05).3.The correlation between UACR level and SYNTAX scoreThere was a significant positive correlation between SYNTAX score and UACR level(r=0.572,P< 0.05).Conclusion: 1.In elderly patients with hypertension.UACR level is a risk factor for coronary heart disease.2.UACR level is positively correlated with coronary artery disease in elderly patients with hypertension and coronary heart disease.UACR can be used as an index to predict the degree of coronary artery disease.
