Study On The Influence Of Psychological Status And Coping Style On The Quality Of Life Of Elderly Patients With Type 2 Diabetes Mellitus Complicated With Retinopathy In Dalian Area

Objective:To analyze the psychological status,coping style and quality of life of type 2diabetes mellitus(T2DM)patients with retinopathy,and to explore the relationship among mental health,coping style and quality of life.Methods : This study was a cross-sectional study.427 patients with type 2 diabetes mellitus complicated with retinopathy(retinopathy after or at the same time diagnosed with diabetes mellitus)were selected.The mental health status,coping style and quality of life of the patients were assessed by symptom checklist 90(SCL-90),medical coping questionnaire(MCMQ)and visual impairment ophthalmopathy quality of life scale.Results : The total score of SCL-90 in male patients with T2 DM combined with retinopathy was(181.11±42.218),and that in female patients with T2 DM combined with retinopathy was(169.71±37.400).The three factors of coping style of patients from high to low were face,avoidance,and yield factors.There were significant differences in the dimensions of face,avoidance and submission,quality of life(physical function,social activity,psychosocial)and mental health of patients with T2 DM and retinopathy between different sexes(p< 0.05).There were significant differences in the face factor,avoidance factor and yield factor of SCL-90 between patients with different educational levels of T2 DM and retinopathy,and the total score of mental health SCL-90 in the quality of life scale(p< 0.05).There were significant differences in face factor and yield factor,total score of mental health SCL-90 and quality of life(visual function,physical function and social activity)between patients with T2 DM and retinopathy with different family monthly income(p<0.05).There were significant differences in the total score of SCL-90 between patients with different courses of T2 DM and retinopathy in the face factor and yield factor of SCL-90(p< 0.05).There were significant differences in yield factor,total score of SCL-90 and quality of life(visual function,social activity,psychosocial)between patients with T2 DM and retinopathy in different family atmosphere(p< 0.05).The total score of SCL-90 was positively correlated with the visual,physical and mentalfields of quality of life in patients with type 2 diabetes mellitus complicated with retinopathy(p< 0.05);in coping style,the factors of face were positively correlated with the visual,physical and mental fields of quality of life(p< 0.01);and the factors of yield were positively correlated with the social and psychological fields of quality of life(p<0.01).Psychological domain was negatively correlated(p< 0.01).Gender(p< 0.05)has statistical significance in the four areas of quality of life;educational level(p< 0.05)has statistical significance in the visual function,social activities and mental and psychological fields of quality of life;family monthly income(p< 0.05)has statistical significance in the social activities and mental and psychological fields of quality of life;family atmosphere(p< 0.05)has statistical significance in the quality of life.The influence of visual function,social activities and mental and psychological fields has statistical significance.Psychological health has a significant direct effect on visual function,body function and mental and psychological fields of patients' quality of life(p< 0.01),and coping face factors have a significant direct effect on all areas of patients' quality of life(p< 0.01).Conclusions:1.Mental health,coping style and quality of life of male patients with T2 DM combined with retinopathy were significantly higher than those of female patients.2.The coping style and mental health of the patients with T2 DM complicated with retinopathy with higher education level are higher than those with lower education level.3.Mental health,coping style(facing,yielding)and quality of life of patients with high monthly income of family with retinopathy were significantly higher than those of patients with low monthly income of family with retinopathy.4.The mental health,coping style(facing,yielding)and quality of life(social activities)of patients with long duration of T2 DM complicated with retinopathy were significantly higher than those of patients with short duration of T2 DM complicated with retinopathy.5.The mental health and quality of life(visual function,social activity,psychosocial)of patients with T2 DM and retinopathy with harmonious family atmosphere were significantly higher than those of patients with T2 DM and retinopathy with contradictory family atmosphere.6.The total score of SCL-90 in patients with T2 DM and retinopathy was positively correlated withthe quality of life(visual function,physical function,mental and psychological);in coping style,face factors were positively correlated with the quality of life(visual function,physical function,mental and psychological fields);and yield factors were negatively correlated with the quality of life(social activity,mental and psychological fields).7.Gender,education level,family atmosphere,mental health and coping style are the influencing factors of quality of life.Active coping can help improve the quality of life of patients.
