| Objective: This article is aimed to explore the factors of unfavourable prognosis of patients with progressive stroke treated with Argatroban. Methods:In this study, all the patients were patients with progressive ischemic stroke who were treated with Argatroban and hospitalized in the first hospital of China Medical University from November 2016 to December 2017.After confirming the patients in the group, collecting clinical data, according to the decrease or increase of NIHSS score, the patients were divided into favourable prognosis group and unfavourable prognosis group.Analyze and compare between the two groups in gender, age, past history(hypertension, diabetes, coronary heart disease, cerebrovascular disease),smoking history, drinking history, biochemical indicators(LDL-cholesterol,D-dimer,fibrinogen,homocysteine,CRP),infarction site, the time of beginning to use Argatroban, whether to take antiplatelet drugs at the same time. Then t-test or chi-square test was used to evaluate the factors of unfavourable prognosis of patients with progressive stroke after using Argatroban. Results: A total of 120 patients were finally included in the study,of which 78 patients(65%)showed favourable prognosis and 42 patients(35%)showed unfavourable prognosis. The single factor analysis showed that there was no significant difference in gender, age, past history(hypertension, diabetes, coronary heart disease, cerebrovascular disease),smoking history, drinking history, biochemical indicators(LDL-cholesterol,D-dimer,fibrinogen,homocysteine,CRP)between the two groups(P>0.05).There was a significant difference between the two groups in the infarction site, the time of beginning to use Argatroban, whether to take antiplatelet drugs at the same time.(P<0.05).Conclusions: The prognosis of patients with anterior circulation infarction is worse than the patients with posterior circulation infarction. The prognosis of patients with progressive stroke treated with Argatroban after 48 hours of onset is worse than the patients treated within 48 hours of onset. The prognosis of patients with progressive stroke treated with Argatroban alone is worse than the patients treated Argatroban with aspirin. |
