| Objective: The purpose of this study is to evaluate the effectiveness of sling exercise therapy(SET)in the improvement of lumbar multifidus(LM)and function in patients with hemiplegia by measuring the cross sectional area(CSA)of LM through ultrasound imaging,measuring the root mean square(RMS)and the median frequency(MF)of LM through surface electromyography(sEMG),provide scientific basis for application of SET,and to explore the clinical application of ultrasound imaging and sEMG.Methods: An assessor-blinded,randomized controlled trial was undertaken in a stroke rehabilitation ward,with 34 participants randomly assigned to an experimental group(n = 18)or a control group(n= 16).The experimental group was treated with routine rehabilitation and sling exercise therapy,referred to as SET group.The control group was treated with routine rehabilitation.The daily training time of the two groups is basically the same.Continuous training for 4 weeks.Before and after the treatment,the cross sectional area(CSA)of LM was measured with ultrasound,the root mean square(RMS)and the median frequency(MF)of LM through surface electromyography(sEMG)and the functional scores of Berg balance scale(BBS),functional independence measure(FIM)were evaluated.Results: CSA: 1.After 4 weeks of treatment,in both group,both sides of CSA of LM were significantly higher than those before the experiment(P <0.05),but they were not significantly higher than those in the control group(P > 0.05).2.In the SET group,the rate of change of the hemiplegic side of the CSA of LM and the ratio rate of the CSA of LM were significantly higher than those in the control group(P <0.05).3.After intervention,the BBS,FIM scores of both groups were significantly higher than those before intervention(P <0.05).But these scores of both groups were not significantly higher than those of the control group(P > 0.05).sEMG :1.After 4 weeks of treatment,in the control group,the difference value of MF of LM were significantly higher than those before the experiment(P <0.05),but nonsense in the SET group(P >0.05)2.In the SET group,the rate of change of MF of LM were significantly higher than those in the control group(P <0.05).Conclusion:1.Sling exercise therapy as same as routine rehabilitation can enhance the strength of multifidus in stroke patients,and more effective in delay the muscles atrophy,reduce the asymmetry,and to improve the balance of stroke patients ability and motor function;2.Ultrasound imagingtechniques and surface electromyography can be used as an objective indicator for the evaluation of rehabilitation training efficacy and providing the reference for the formulation of the rehabilitation plan;... |
