The Correlation Research Of Lumbar Spondylolisthesis Andendal Ate Modic Changes

ObjectivesTo investigate the distribution characteristics of endplate Modic changes in patients with lumbar spondylolisthesis and summarize the distribution of Modic changes,and to investigate the correlation between lumbar spondylolysis and endplate Modic changes.MethodsThe MRI data of 129 patients with lumbar spondylolysis in our hospital from January 2014 to October 2017 were analyzed retrospec-tively and 78 normal spondylol isthesis of lumbar spine with the same age and sex were selected as the normal group.The incidence and distribution of various types of Modic were recorded and statistically analyzed,and the degree of lumbar spondylolisthesis and intervertebral disc degeneration in lumbar spondylolysis were observed.To observe the relationship between endplate Modic change and lumbar spondylolisthesis and intervertebral disc degeneration in patients with lumbar spondylolysis,and to investigate the correlation between lumbar spondylolysis and endplate Modic changes.Results129 cases of lumbar spondylolisthesis group,110 cases of Modic change,normal group 68 cases,10 cases of Modic changes,Modic changes in the two groups have statistical significance.Modic changes in the isthmus group were 48 males and 67 females.There were 3 males and 7 females in the normal group.There was no statistically signifycant difference between different sexes.The incidence of Modic change in patients under 40 years of age and over 40 years old in spondylolisthesis patientswas 77.8% and 92.8%,respectively,and the difference was statistically significant(p<0.05).There were 137 spondylolisthesis vertebral layers in 129 patients with isthmus fissure group,of which 96 were L5 and 41 were L4,The incidence of Modic changes was 87.5% and 95.1%,respectively,and 1 cases were in L2,and there were Modic changes.There were 13 cases of unilateral spondylolysis and 116 cases of bilateral spondylolysis,and the incidence of Modic change was 69.2% and 91.4% respectively,with statistical difference(p<0.05);According to Meyerding grading system,92 were in grade I and 11 in gradeⅡ,and no grade Ⅲ,Ⅳof spondylolisthesis was found.The incidence of spondylolisthesis in unilater-al and bilateral spondylolysis was 15.4% and 87.1%,respectively,and the difference betw-een the two groups was statistically significant.There was a statistically significant difference in the degree of degeneration between the upper and lower intervertebral discs in the isthmic fracture group(p<0.01),and the degree of disc degeneration below the lumbar spondylolysis was significantly higher than that in the upper intervertebral disc degeneration.There was a significant difference between the different groups of Modic and no Modic changes in the spondylolysis of the lumbar spondylolisthesis(p<0.05).There was significant difference in the degree of spondylolisthesis and intervertebral disc degeneration,and the degree of spondylolisthesis was correlated with the degeneration of intervertebral disc.Among 129 cases of lumbar spondylolysis,26 cases had no spondyl-olisthesis and 103 cases had spondylolisthesis.The incidence of Modic change in the nor-mal group,the isthmic spondylolisthesis without spondylolisthesis group and the isthmic spondylolisthesis group was 14.7%,69.2% and 89.3%,respectively,and the difference was statistically significant(p<0.01).Among 1290 endplates of 129 patients with lumbar spondylolisthesis,and 110 cases of 253 endplates were involved in Modic changes,Amo-ng these,8 endplates were classified as type I,212 endplates were classified as type II,26 endplates were classified as type III,and 7 endplates were classified as mixed type;the lesions involved L1/2 segment in 18,L2/3 segment in 26,L3/4 segment in 28,L4/5 segm-ent in 50,L5/S1 segment in 131.The incidence of Modic changes in the adjacent end plates of non-slippage segements and spondylolisthesis was 10.2% and 66.2%,respective-ly.The incidence of Modic change was higher in spondylolisthesis than in non spondylol-isthesis segments,and the difference was statistically significant(p<0.01).The incidence of Modic changes in spondylolisthesis in patients with spondylolisthesis in spondylolisth-esis,1 degree slipping segmental and second degree spondylolisthesis was 10.2%,64.1% and 88.9%,respectively,Spearman correlation test showed that there was a correlation between the Modic change of the lumbar endplate and the degree of lumbar spondylolisthe-sis(r=0.529,p<0.01).Conclusion1.The incidence of Modic changes in the lumbar spondylolisthesis group was higher than that in the normal group.2.The degree of degeneration of the intervertebral disc under the spondylolisthesis is significantly higher than that of the upper intervertebral disc,and the degree of disc degen-eration is associated with the spondylolisthesis.The degree of lumbar spondylolisthesis and lumbar spondylolisthesis are correlated.3.The Modic change of the lumbar endplate is correlated with the degree of lumbar spondylolisthesis.4.In patients with lumbar spondylolysis,endplate Modic changes were more comm.-on in type II,and Modic changes occurred mostly in the spondylolisthesis segment.
