| Purpose:This study is aimed at a study on therapeutic value of vacuum sealing drainage(VSD)in traumatic soft tissue defect of extremities,and comparing the difference in therapeutic effects on soft tissue defect of extremities between the vacuum sealing drainage and the traditional therapeutic method.In order to promote further development of China’s therapy technique of skin and soft tissue defect,it is expected that this technique can be popularized in the basic medical institutions,benefiting a broad range of patients.Method:Collecting the patients of skin and soft tissue defect of extremities caused by trauma received by trauma limbs surgery of Tai’an City Central Hospital from January 2017 to February 2018.In which,32 cases of skin and soft tissue defect patients treated by the vacuum sealing drainage were the experimental group;32 cases of skin and soft tissue defect patients treated by the traditional dressing change were the control group.Exclusion criteria: exposed deep tissue such as bone,joint,and important vascular nerve,along with the basic diseases preventing wound healing such as severe diabetes,hemorrhagic disease.The clinical effects of the two therapeutic methods were evaluated,by recording the data such as average hospital stay of the patients in each group after treatment,time between first debridement and surgical treatment,times of dressing change,VAS pain score at Day 3,7,and 11 after the debridement,hospitalization expense,survival condition of skin graft,and conducting statistical processing for the recorded data by using SPSS22.0 statistical software.Results:1.The general data factors in the two groups were balanced,and distributions of sex composition,age,skin and soft tissue defect area of the patients had no significant difference by a statistical test(P>0.05).2.The times of dressing change in the VSD experimental group was(1.91±0.78),and times of dressing change in the control group was(9.72±2.04),the difference between the two groups was statistically significant(P<0.05).The time between first debridement and secondary operation in the VSD experimental group was(7.84±2.36)d,and the time between the first debridement and the secondary operation in the control group was(14.03±3.65)d,the difference was statistically significant(P<0.05);the average hospital stay of the patients in the experimental group using the VSD technique was(27.44±3.38)d,being evidently less than(34.30±9.06)d of the control group,the difference between the two groups was statistically significant(P<0.05).3.The VAS pain scores at Day 3,7,and 11 after the first debridement in the VSD experimental group were lower than those of the control group of traditional treatment.Statistical analyses were conducted for the VAS pain scores at the corresponding time after treatment in the two groups,the difference was statistically significant,the further details are: at Day 3 after the debridement(t=4.05,P<0.05),at Day 7 after the debridement(t=4.85,P<0.05),at day 11 after the debridement(t=9.39,P<0.05).4.The average hospitalization expense of the patients in the VSD experimental group was(36282.57±13458.51)RMB,the hospitalization expense in traditional treatment control group was(32507±14507.38)RMB.The difference between the two groups was not statistically significant(P>0.05).5.In this study,4 cases of the patients of 32 cases of the patients in the experimental group were subjected to secondary suture,and 28 cases were subjected to secondary free skin grafting,and continued to be treated by using the VSD technique after the skin grafting;2 cases of the patients of 32 cases of the patients in the control group were subjected to the secondary suture,and 30 cases of the patients were subjected to secondary free skin grafting,this group were treated by traditional package compression bandage after the skin grafting.Skin graft survival rate of 28 cases of the patients in the VSD experimental group was(93.64±3.55)%,and skin graft survival rate of 30 cases of the patients in the control group was(85.92±6.14)%,the difference between the two groups was significant,and it was statistically significant(P<0.01).Conclusion: The VSD technique can evidently shorten the wound healing time,and enable the patients to receive secondary surgical treatment as early as possible,reducing times of the dressing change,so as to shorten the hospital stay and relieve pain degree at the wound of the patient,and increase the skin graft survival rate. |
