The Application Value Of Resecting The Positive Margin Of Cervical Lesion After The Operation

Objective:To compare and evaluate the clinical efficacy and medical value of retractable pyramidectomy and total hysterectomy as supplementary treatment for patients with positive margin after cervical conicectomy,and to confirm the necessity of two supplementary surgeries for patients with positive margin.Methods:The clinical data of219 patients with positive margin after cervical coning resection in our hospital from July2010 to January 2020 were retrospectively analyzed,which were divided into the primary cervical coning group,the second cervical coning group and the total hysterectomy group,and the clinical data of the patients were analyzed.SPSS 19.0 statistical software was used for analysis.χ~2 test was used for counting data.The measurement data were analyzed by t test or variance analysis.Results:There were statistically significant differences in the residual recurrence rate(40.4%)between the supplementary operation group(second cervical coning and total hysterectomy)and the primary cervical coning group(P<0.05),and no statistically significant differences between the second cervical coning group(8.3%)and the total hysterectomy group(9.5%)(P>0.05).The average hospitalization cost of the first cervical cone resection group was 7,260.32±3,4330.79 yuan,the average hospitalization cost of the second cervical cone resection group was 5,666.56±1702.00yuan,and the average hospitalization cost of the total uterine resection group was 2,090.11±9812.59 yuan.There was no statistical difference between the two cervical coning groups(P>0.05),which was statistically significant compared with the total hysterectomy group(P<0.001).The average reimbursement amount was 3,825.72±2045.97 yuan in the two cervical coning surgery groups and 15,780.37±9560.51 yuan in the hysterectomy group,with statistically significant differences(P<0.001).Conclusion:In addition to safety and effectiveness,cervical conectomy has many advantages,such as small wound area,quick postoperative recovery,short hospital stay,low medical cost and light medical insurance burden.However,accurate preoperative assessment of the disease and accurate intraoperative surgical operation are important guarantees for the success of the operation.
