ObjectivesSleep quality was associated with health,and coronary heart disease was a major disease affecting health.Although studies have been conducted to explore the association between sleep quality and coronary heart disease,the evidence in China was scarce.The aims of the study were to explore the prevalence of poor sleep quality and the association between sleep and coronary heart disease among Henan rural adults.MethodsThere were 27935 participants in this analysis recruited from "Henan Rural Cohort Study"(ChiCTR-OOC-15006699).Age of the participants ranged from 18-79 years old.Face-to-face interviews were administered by well-trained staffs to obtain information through a structured questionnaire.Information about sleep was obtained through Pittsburgh Sleep Quality Index(PSQI).The score>5 indicated poor sleep quality.Logistic regression models were used to identify the association between sleep and coronary heart disease.Restricted cubic regression splines were conducted to analyze the association between continuous sleep score and coronary heart disease.Results1.In the total population,the average age was 55.96(12.22)years old.The average sleep score was 3.79(2.73).Average sleep score of women was 4.12(2.91)which was significantly higher than men 3.30(2.35).The overall crude and age-standardized prevalence of poor sleep quality were 21.80%and 15.88%.Crude and age-standardized prevalence of women were 25.95%and 18.08%.Crude and age-standardized prevalence of men were 15.59%and 12.62%.The prevalence of coronary heart disease increased with the increasing age.Sleep score of participants suffering coronary heart disease was 4.96(3.25),which was significantly higher than those without coronary heart disease 3.72(2.68).2.Compared with participants of good sleep quality,the OR(95%CI)of coronary heart disease of participants with poor sleep quality was 2.10(1.89-2.35)in the crude model.After adjusting full covariates,the association remained significantly,and the OR(95%CI)was 1.67(1.49-1.88).The association remained in both genders.3.In the stratification analysis,subjects who were younger than 60 years old,?1000 yuan/month income and without depression had higher ORs of coronary heart disease when exposed to poor sleep quality,and the OR(95%CI)were 1.14(1.11-1.17),1.14(1.11-1.18)and 1.11(1.09-1.13).In the men,the stronger effect of poor sleep quality on coronary heart disease were found among subjects of<60 years old and without depression,and the OR(95%CI)were 1.14(1.07-1.21)and 1.10(1.06-1.14).While in the women,the stronger effect of poor sleep quality on coronary heart disease were found among subjects under 60 years old,? 1000 yuan/month income and BMI<24kg/m2,and the OR(95%CI)were 1.14(1.10-1.17),1.15(1.11-1.20)and 1.14(1.10-1.18).4.In the male,poor subjective sleep quality,longer sleep latency,sleep disturbance and using of sleep medicine were associated with the prevalence of coronary heart disease.In the female,poor subjective sleep quality,longer sleep latency,lower sleep efficiency,sleep disturbance,using of sleep medication and daytime dysfunction were related to the prevalence of coronary heart disease.Furthermore,compared with women with 7-8h sleep duration,the OR(95%CI)of women with sleep duration<5h was 1.62(1.08-2.42).Conclusion1.Poor sleep quality was associated with the prevalence of coronary heart disease among Henan rural adults.2.The stronger effect of poor sleep quality on coronary heart disease was found among participants of<60 years old,? 1000 yuan/month income and non-depression.3.Apart from overall sleep quality,the components of sleep were also significantly associated with coronary heart disease.
