BackgroundAlopecia areata(AA)is a common non-cicatricial hair shedding disorder in dermatology clinics.The majority of the patients were children and middle-aged and young adults,and the incidence had an increasing trend.Clinical manifestations vary from sudden patchy hair loss to Alopecia Totalis(AT)with total hair loss,even Alopecia Universalis(AU)with hair,eyebrows,eyelashes,armpit hair,pubic hair,and body hair loss.For mild cases,the prognosis is better.However,the therapeutic effect of refractory AA,AT and AU is often unsatisfactory,and the recurrence rate is high.The etiology and specific pathogenesis of this disease are not very clear,and a number of studies have shown that AA may belong to the category of autoimmune diseases.At present,AA is considered to be an autoimmune disease mediated by T cells and specific to hair follicle organs.A variety of cells and cytokines may play an important role in the pathogenesis of AA.Tregs(regulatory T cells)play an important role in maintaining immune tolerance and balance,suppressing immune response and related immune diseases.Recent studies have found that Treg cells in AA patients have abnormal number or function,which may be involved in the pathogenesis of AA.INF-? is an inflammatory factor mainly secreted by T cells,NK cells,etc.,which is considered to be involved in the pathological process of AA and one of the key factors in AA,playing a crucial role in improving the expression of MHC-I class,promoting antigen recognition and the collapse of hair follicle privilege.IL-10 is an anti-inflammatory factor,mainly secreted by Th2 cells,Tregs and other cells,which plays a certain role in inhibiting Th1 function and maintaining Th1/Th2 balance.Currently,there are few studies on its relationship with AA.The relationship between Treg cells,IL-10 and AA is not fully understood,and the detailed mechanism of INF-y and AA is yet to be further studied.The purpose of this study was to study the absolute value of Treg cells in peripheral blood and the serum concentrations of INF-? and IL-10 in AA patients,to explore the relationship between them and AA,to further understand the pathogenesis of AA,and to contribute to the development of effective drugs for the treatment of AA.ObjectiveBy detecting the absolute value of peripheral Treg cells,serum levels of INF-?and IL-10 in AA patients,the relationship between they and AA was discussed,and the pathogenesis of AA was further improved.MethodsSamples of 22 AA cases will be collected,divided into mild group and severe group,the Active and inactive group,?6 months group and>6 months group,flow cytometry method is applied to detect patients with AA Treg cells in peripheral blood of absolute value,Serum INF-? and IL-10 concentrations in AA patients were determined by flow microsphere array,and comparing with 21 cases of healthy physical examination.To compare the differences among AA patients with different degrees of severity,different courses of disease and whether the disease is active or not.SPSS23.0 software was used for statistical analysis.The data were expressed as mean ± standard deviation,and P<0.05 was considered statistically significant.Spearman test was used for correlation analysis.Results1.The absolute value of Treg cells(1)The absolute value of peripheral blood Treg cells in AA group(71.95±37.31/ul)was significantly higher than that in the control group(36.08±17.32/ul),and the difference was statistically significant(P<0.05).(2)the absolute value of peripheral blood Treg cells(77.66±37.41/ul)in severe AA group was higher than that in the control group,and the difference was statistically significant(P<0.05).The severe group was higher than the mild group(69.09±42.39/ul),and the mild group was higher than the control group(P>0.05).The differences were not statistically significant.(3)the absolute value of peripheral blood Treg cells of AA patients in the active group(70.96±31.81/ul)was higher than that in the inactive group(67.73±48.42/ul),and that in the active group(36.08±17.32/ul)was higher than that in the control group(P>0.05).(4)the absolute value of peripheral blood Treg cells in patients with alopecia areata in the duration ?6 months group(70.19±45.73/ul)was higher than that in patients with AA in the 6 months group(68.20±22.30/ul),and that in patients with AA in the 6 months group(36.08±17.32/ul),with no statistically significant difference(P>0.05).(3)the absolute value of peripheral blood Treg cells of AA patients in the active group(70.96±31.81/ul)was higher than that in the inactive group(67.73±48.42/ul),the active group was higher than that in the control group(36.08±17.32/ul),and the inactive group was higher than that in the control group,The differences were not statistically significant(P>0.05).(4)The absolute value of peripheral blood Treg cells(70.19±45.73/ul)in patients with AA with a course of?6 months is higher than that in patients with AA in>6 months group(68.20±22.30/ul),and that in patients with AA in>6 months group is higher than the control group(36.08±17.32/ul),? 6 months group was higher than the control group,the difference was not statistically significant(P>0.05).2.The level of IFN-y(1)the Serum IFN-y level in AA group(1.50±0.33pg/ml)was significantly higher than that in the control group(1.18±0.33pg/ml),and the difference was statistically significant(P<0.05).Serum IFN-y level of AA patients in the severe group(1.58±0.44pg/ml)was higher than that in the control group(1.18±0.33pg/ml),and the difference was statistically significant(P<0.05).The severe group was higher than the mild group(1.32±0.26pg/ml),and the mild group was higher than the control group,the differences were not statistically significant(P>0.05).(2)the serum IFN-y level of AA patients in the active group(0.88±0.10pg/ml)was lower than that in the inactive group(1.08±0.34pg/ml),the inactive group was lower than that in the control group(1.18±0.33pg/ml),and the active group was lower than that in the control group,the differences were not statistically significant.(3)the serum IFN-y level of patients with AA in the duration ?6 months group(1.23±0.27pg/ml)was higher than that in the>6 months group(1.20±0.41pg/ml).The IFN-y level of patients with AA in the>6 months group(1.18±0.33pg/ml)was higher than that in the control group(1.18±0.33pg/ml),and that in the ?6 months group was higher than that in the control group,the differences were not statistically significant(P>0.05).3.The level of IL-10(1)The serum IL-10 level in the active group(0.75±0.20pg/ml)was lower than that in the inactive group(1.02±0.14pg/ml)and the control group(1.13±0.23pg/ml),with statistically significant differences(P<0.05).The inactive group was lower than the control group,and the difference was not statistically significant(P>0.05).Serum IL-10 level(1.02±0.21pg/ml)in AA group was lower than that in the control group,with no statistically significant difference(P>0.05).(2)The serum IL-10 level in the mild group(1.01±0.18pg/ml)was lower than that in the severe group(1.08±0.26pg/ml),the severe group was lower than the control group(1.13±0.23pg/ml),and the mild group was lower than the control group,the differences were not statistically significant(P>0.05).The serum IL-10 level in the ?6 month group(0.97±0.13pg/ml)was lower than the>6 month group(1.07±0.28pg/ml),the>6 month group was lower than the control group(1.13±0.23pg/ml),and the ?6 month group was lower than the control group,the difference There was no statistical significance(P>0.05).4.Correlation analysis of INF-? and IL-10Serum INF-? was positively correlated with IL-10 in alopecia areata group(r=0.729,p=0.01).Conclusion1.The absolute value of Treg cells and the level of IFN-y in patients with AA group were significantly higher than those in the control group,and the levels of both severe groups were significantly higher than those in the control group,It is speculated that both of them may be involved in the incidence of AA and are related to the severity of AA,but may not be related to the disease course and activity of AA.2.The level of IL-10 in the active AA group was significantly lower than that in the inactive group and the control group,which may be related to the activity of AA.3.Serum IFN-y was positively correlated with IL-10 level in AA group,suggesting that both were involved in AA.
