| Objective:The purpose of this study is to analyze and summarize the current situation of contracted services of family doctors in the urban area of Changchun City,and to investigate the contracted services of family doctors in 10 community health service centers in 7 administrative regions of Changchun City.This study uses two perspectives of both supply and demand to understand the contracted service level of family doctors provided by primary medical institutions in Changchun City,exploring the existing shortcomings to provide reference suggestions for further improving the contracted service ability of family doctors in Changchun City.Methods:This paper adopts literature reading method,questionnaire survey method and interview method for research and analysis.Read literature to comprehend the implementation of contracted services of foreign family doctors.Sampling survey was conducted,with 2839 valid questionnaires were collected from residents by using “The Questionnaire of Contracted Service of Changchun City Resident Family Doctors”as investigative tools,and 326 valid questionnaires were collected form family doctors by using “The Questionnaire of Contracted Services of Family Doctors in Changchun City”as investigative tools.The database was established and analyzed by SPSS20.0.In addition,the author interviewed the family doctors of Changchun Community Health Service Center and summarized the conversation.Results: The findings of investigation indicated that 67.5% of the residents who went to a doctor have signed contracts with family doctors,with the remaining 32.5% of the residents did not sign the contract.The main reasons for not signing a family doctor are that the residents do not know or understand the services signed by the family doctor,and they do not trust the professional ability of family doctors.Besides,some of the residents are prefer to practice their own habits of consultation.62.9% of the clients who have signed the contract were introduced by community doctors,which shows that residents have less initiative to complete the contract on their own initiative.Therefore,the awareness andtrustiness of residents for family doctors’ contract services need to be enhance to improve the initiatives.The results of the difference in signing contracts of residents in different groups shows that,the age,education level and self-rated health of residents are statistically significant(P<0.05).According to the current statistical analysis,the older the age,the higher the contract rate.As mentioned in the interview,the family doctor services provided by the community health service centers preferentially cover the elderly.In terms of residents’ self-rated health status,the households with poor self-rated health had the highest rate of signing contracts,reaching 77.46%.In terms of willingness to first visit in the community,P>0.05,which indicates that Chi-square test showed no statistical significance.It can be seen that the signing status of family doctors has no relationship with the attitude of first visit in the community,and residents’ evaluation of community health service centers shows a general level of satisfaction.The survey result of family doctors and their related shows that the main problems faced by family doctors in Changchun at present are the insufficient number of relevant health service personnel,inadequate hardware facilities,the difficulty in implementing the appointment diagnosis and treatment system,and the difficulty in meeting the needs of community residents in the current grassroots service capacity.By investigating the family doctors and other relevant health personnel,the conclusion has been drawn.At present,the main problems faced by family doctors signing services in Changchun are the insufficient in number of relevant health service personnel,inadequate in conditions of corresponding hardware facilities,the capacity of appointment referral service needs to be improved,so that the current primary service capacity of family doctors could not meet the needs of community residents.Conclusion:From a policy perspective,relevant departments in Changchun promulgated the standards of the management of family doctors’ contracted services,incorporating the rate of contracted services of family doctors into the evaluation criteria of primary medical institutions.But even with the high level of signing rate,the reality is thatmost of the residents signed contracts but do not perform them.In order to ensure that the family doctor contract system is well implemented,the first is to strengthen the service capacity of primary medical institutions,as well as establish the corresponding performance evaluation system and incentive policies,and the second is to improve the awareness and trustiness of community residents,as well as to change the residents’ habits of consultation and concepts of medical treatment. |
