Objective:To observe the clinical efficacy of Modified Ermiao Formula in the prevention and treatment of HR-HPV persistent infection after LEEP in SIL patients, and to evaluate the effectiveness and safety of this experience prescription in preventing and treating postoperative HR-HPV infection. In order to reduce the postoperative HPV infection and prevent the recurrence and progression of SIL,provide new ideas for TCM treatment.Method:From January 2018 to March 2019,60 patients with HR-HPV positive preoperatively and TCM syndrome of dampness-heat diffusing downward were selected from patients who underwent LEEP treatment at the gynecological department of Jiangsu Provincial Hospital of TCM.They were divided into treatment group and control group with 30 cases each. The treatment group was treated with Modified Ermiao Formula, and the control group was followed up for postoperative observation. The clinical efficacy was evaluated after 6 months of treatment.Results:(1)Postoperative HR-HPV negative rate:The HR-HPV negative rate in the treatment group was 82.76%(24/29),and the HR-HPV negative rate in the control group was 58.62%(17/29).The HPV negative rate was significantly better than the control group, and the difference between the groups was statistically significant(P<0.05).(2)Postoperative negative rate of different HPV infections:The negative rate of single HPV infection was 84.21%(16/19)in the treatment group and the negative rate of 82.35%(14/17)in the control group, the negative rate of single HPV infection was similar in the two groups, and the difference was not statistically significant(P>0.05);compared with the negative rate of double HPV infection,The negative rate of negative HPV infection in the treatment group was 88.89%(8/9),and the negative rate of negative HPV infection in the control group was 30.00%(3/10).The negative rate of double HPV infection was better in the treatment group than in the control group, and the difference was statistically significant(P<0.05).(3)Postoperative negative rate of different HPV infection types:Comparison of type 16 HPV negative rate, the negative rate in the treatment group was 77.78%(7/9),and the negative rate in the control group was 60.00%(6/10);comparison of type 18 HPV negative rate, the negative rate in the treatment group was 66.67%(2/3),and the negative rate in the control group was 50.00%(2/4);comparison of type 33 HPV negative rate, the negative rate in the treatment group was 100.00%(3/3),and the negative rate in the control group was 50.00%(1/2);comparison of type 52 HPV negative rate, the negative rate in the treatment group was 100.00%(6/6),and the negative rate in the control group was 45.45%(5/11);comparison of type 56 HPV negative rate, the negative rate in the treatment group was 100.00%(1/1),and the negative rate in the control group was 33.33%(1/3);comparison of type 58 HPV negative rate, the negative rate in treatment group was 100.00%(5/5),and the negative rate in the control group was 50.00%(1/2).In the treatment group, the rate of negative conversion in type 16,18,33,52,56 and 58 infections was higher than that in the control group, but the differences between the groups were not statistically significant(P>0.05)(4)Postoperative HPV negative rate of different pathological levels:Comparison of HPV negative rate in chronic cervicitis, the negative rate in the treatment group was 100.00%(2/2),and the negative rate in the control group was 50.00%(1/2);comparison of HPV negative rate in LSIL,the negative rate in the treatment group was 81.82%(9/11),and the negative rate in the control group was 50.00%(5/10);comparison of HPV negative rate in HSIL,the negative rate of negative rate in the treatment group was 81.25%(13/16),and the negative rate of negative rate in the control group was 64.71%(11/17).The postoperative HPV negative rate in chronic cervicitis,LSIL,and HSIL in the treatment group was higher than that in the control group, but there was no significant difference between the groups(P>0.05)(5)Cervical cytological results:The cytological abnormality rate in the treatment group was 3.45%(1/29),and the cytological abnormality rate in the control group was 13.79%(4/29).The abnormal results were all ASC-US.The cytological abnormality rate in the treatment group was lower than that in the control group, and the difference was not statistically significant(P>0.05).(6)Scores of TCM symptoms:Intra-group comparison in two groups, the scores of TCM syndromes after treatment were significantly lower than before treatment, and the difference was statistically significant(P<0.01);Compared between groups, the improvement of TCM syndrome scores in the treatment group was significantly better than that in the control group, and the difference was statistically significant(P<0.01).(7)The total effective rate of clinical efficacy:The total effective rate in the treatment group was 93.10%(27/29),and the total effective rate in the control group was 65.52%(19/29).The total clinical effective rate in the treatment group was significantly higher than that in the control group, and the difference between the groups was statistically significant(P<0.05).Conclusion:(1)Modified Ermiao Formula can prevent persistent HR-HPV infection after LEEP in SIL patients with syndrome of dampness-heat diffusing downward, significantly improve the TCM syndromes of patients, safe and effective, and worthy of further clinical application.(2)The effect of Modified Ermiao Formula on the prevention and treatment of persistent HR-HPV infection in postoperative is effective in the double HPV infection.
