BackgroundCoronary atherosclerotic heart disease(CHD)has become a global public health problem and tends to be younger and rural.As a province with a large population,Henan suffers from an increasing number of coronary heart disease,and the prevention and treatment of coronary heart disease must not be delayed.A large number of studies have confirmed that plasma fibrinogen(Fib)can be used as a coagulation factor and proinflammatory factor to participate in the occurrence and development of coronary heart disease.However,the influence of plasma fibrinogen in rural population in north Henan on the type and degree of disease of patients with coronary heart disease is not clear.ObjectiveTo study the effect of plasma fibrinogen on the disease type and degree of disease in patients with coronary heart disease in rural areas in north Henan,and to determine the cut-off value of plasma fibrinogen in rural population in north Henan.MethodFrom January 2016 to June 2018,a total of 2699 patients in rural areas of north Henan who underwent coronary angiography were selected from the Henan Engineering Research Center for Clinical Data and Biobank of Cardiovascular Diseases.According to the results of coronary angiography,it was divided into coronary heart disease group and non-coronary heart disease group.The coronary heart disease group was classified according to the number of lesions,the degree of disease,and the type of disease.Binary Logistic regression was used to analyze the risk factors of CHD in the rural population of north Henan.Propensity Score Match(PSM)was used to eliminate the differences betweenthe groups.Multiple logistic regression analysis of the relationship between fibrinogen and the type of coronary heart disease and the degree of coronary artery disease in the rural areas of north Henan.Result1.Compared with the non-coronary heart disease group,the age,sex,smoking,drinking,diabetes history,history of hypertension,history of dyslipidemia,and plasma fibrinogen were significantly different in the coronary heart disease group(P<0.05).2.The results of the binary logistic regression model suggested that plasma fibrinogen(P <0.05,OR 1.002,95% CI 1.001-1.004)was an independent risk factor for coronary heart disease.3.The specific cut-off value of plasma fibrinogen is 313.3 mg / dl,and its sensitivity to the diagnosis of coronary heart disease is 24.2% and its specificity is 84.2%.4.Taking the plasma fibrinogen concentration of 313.3mg / dl as the cut-off value and matching the propensity scores,the incidence of coronary heart disease in the group with fibrinogen above 313.3mg / dl was significantly higher than that with fibrinogen below313.3mg / dl.5.Ordered Logistic regression analysis showed that plasma fibrinogen(P <0.05,OR0.63,95% CI 0.5-0.8)was an independent influencing factor of the number of coronary heart disease lesions.6.Multiple logistic regression analysis showed that patients with plasma fibrinogen higher than 313.3 mg / dl were 2.71 times more likely to have moderate coronary heart disease than patients with fibrinogen lower than 313.3 mg / dl,and their severe coronary disease was likely to be lower than the 313.3 mg / dl group 1.96 times.7.Multivariate Logistic regression analysis showed that the people with plasma fibrinogen higher than 313.3 mg / dl had a 3.45 times higher probability of acutemyocardial infarction than those with fibrinogen lower than 313.3 mg / dl,and their probability of unstable angina was lower than 313.3 mg / dl group 1.51 times.ConclusionPlasma fibrinogen concentration can reflect the severity of coronary artery disease in rural population in north Henan,and plasma fibrinogen is 313.3mg / dl is the cut-off value for the diagnosis of coronary heart disease in north Henan.Fibrinogen concentration is greater than 313.3mg / dl is Independent risk factors for coronary heart disease.
