| Objective: To analyze the characteristics of electrogastrogram and heart rate variability in patients with type 2 diabetes and type 2 diabetes combined with hyperlipidemia, and to study the gastric rhythm and autonomic nerve activity and their correlations of these patients,to explore possible mechanisms. Methods: 35 cases of type 2 diabetes with normal blood lipids(group1)and 35 cases of type 2 diabetes with hyperlipidemia(group 2)were collected in the Department of Gastroenterology and Endocrinology of Hainan Provincial People’s Hospital.35 cases of healthy volunteers were selected as the normal control group, and the above subjects were informed of their informed consent. The three groups of subjects were given conventionally diet control, appropriate exercise and necessary basic hypoglycemic treatment.Drugs of affecting gastrointestinal motility, heart rate, blood lipids, and autonomic nerve activity were banned.Smoking, tea, alcohol, and coffee were prohibited 12 hours before the test,to avoid violent Activity, maintain adequate sleep, avoid excessive mood swings, and received electrocardiogram and heart rate variability examination after fasting 8 hours in the early morning.Record fasting and postprandial gastric electrical activity parameters and heart rate variability parameters.SPSS 25.0 statistical software was used to statistical processing the obtained data,to investigate whether the characteristic of electrocardiogram parameters and heart rate variability of patients with type 2 diabetes and type 2 diabetes with hyperlipidemia, and to explore the relationship between the gastric electrical rhythm and autonomic nerve function of these patients.Results:1.Analysis of Gastric Rhythm in Three Groups of Subjects:(1)Test group: the ratio of In normal range of two test groups before and after meal is less than 70%,and the changes before and after meals are not obvious. There is no obvious regularity in the change of Dominant frequency and Dominant power between channels, and there is no statistical difference between meals before and after meals. Both the proportion and the rate of yachygastria increased, and the proportion of yachygastria increased significantly.(2)Normal control group: The proportion of in normal range before and after meals in the normal control group were greater than 70%,and increased after meals compared to before meals, which was obvious at 3 and 4 channels, the difference was statistically significant(P<0.05);dominant frequency and dominant power in the order of 1-4 channels are increasing. The Dominant frequency before and after meals is greater than 1,while the rate of bradygastria, yachygastria, and arrhythmic are lower.2.Comparison of gastric electrical parameters among three groups of subjects: There were significant differences between the three groups of study subjects before and after meals, including the proportion of in normal range, the rate of yachygastria, the rate of bradygastria, and the dominant frequency(P<0.05).Multiple comparisons between groups showed that the ratio of in normal range before and after meals in the two experimental groups were lower than that in the normal control group, and the ratio of in normal range after meals in the two experimental groups was lower than that in the experimental one.The above differences are statistically significant(P<0.05);the proportion of bradygastria and yachygastria before and after meals in the two experimental groups was higher than that in the normal control group, and the difference was statistically significant(P<0.05);the proportion of arrhythmic after the meal in the test 1 group was higher than the normal control group, and the difference was statistically significant(P<0.05);the pre-meal dominant frequency of the two experimental groups was lower than that of the normal control group, and the difference was statistically significant(P<0.05).The post-meal dominant frequency of the three groups of study subjects were compared in pairs, and the differences were statistically significant(P<0.05),and the test 2 group was lower than the test 1 group and the normal control group;the pairwise comparison of the other indicators was not statistically different.3.Analysis of heart rate variability: The comparison of autonomic nerve activity,low-frequency power and high-frequency power of the three groups of research subjects showed statistically significant differences(P<0.05).After multiple comparative analysis, the autonomic nerve activity,low-frequency power, and high-frequency power of the two test groups were lower than that of the control group, and the difference was statistically significant(P<0.05),in which the difference between the high-frequency power and the control group was obvious; There was no statistical difference between the heart rate variability indexes of the two test groups.4.Correlation analysis of In normal range and autonomic nerve function: The results of Spearman correlation analysis showed that there was a positive correlation between the proportion of In normal range in the test group 1 and autonomic nerve activity(P<0.05),and In normal range in the test group 2 There was no correlation between ratio and autonomic activity. Conclusion: 1.Abnormal gastric electrical rhythm is common in patients with type 2 diabetes.Abnormal gastric electrical rhythm is more obvious in patients of type 2 diabetes with hyperlipidemia, mainly due to the decrease in the in normal range and the increase in the yachygastria, resulting in clinical gastric delayed emptying;2.Patients with type 2 diabetes and type 2 diabetes combined with hyperlipidemia have autonomic dysfunction, mainly due to decreased parasympathetic activity;3.The in normal range of type 2 diabetes patients was positively correlated with autonomic nerve activity. |
