Objective: Many studies have confirmed that platform-transfer implants reduce bone resorption in the neck of implants.The principle of platform transfer is to move the implant-base junction to the mid-axis of the implant by placing smaller-diameter base tables on larger implants.therefore,inflammatory cell infiltration always exists at the implant-base interface and maintains a certain distance from the bone,which can prevent or reduce alveolar bone loss.However,studies have also shown that platform-transfer implants have a limited effect on cervical bone resorption,whereas implants with internal and external connections with smooth neck have a more stable surrounding soft and hard tissue.The purpose of this study was to compare the effects of Thommen internal and external connected implants and Osstem internal connected platform transfer implants on the surrounding soft and hard tissues,and to provide a reference for the clinical application selection of different connected implants.Methods: This study reviewed a total of 40 patients with tooth loss treated in our hospital from September 2017 to December 2019.Of these,22 were male and 18 were female.The mean age was 49.33 years(20-81 years).A total of 96 implants were implanted.A total of 40 implants were included in the experiment.The patients were divided into two groups according to the different ways of implant connection.A group included a total of 20 implants,the implant used was Osstem internal connection implant,B group included a total of 20 implants,and the implant used was Thommen internal and external connection implant.The implants included in all the experiments were located in the posterior tooth area and were completed by routine implantation.After 3-4 months,6-8 months and 12-14 months,the curved surface tomography andapical X were taken.The bone resorption value of implant neck margin was calculated by measuring and analyzing software,postoperative pain was evaluated,implant stability coefficient(ISQ),modified plaque index,bleeding index were measured and the success rate,mechanical complications and implant satisfaction were measured.Results: The results showed that there was no significant difference the two groups after operation.At 3 months after operation,the ISQ value of B group was larger than that of A group,and the difference was statistically significant.The marginal bone resorption in A group was 0.26±0.05 mm,B and the marginal bone resorption was0.24±0.04 mm in the follow-up stage 3-4 months after operation.There was no significant difference between the two groups.Nevertheless,after 6-8 months and12-14 months of implant loading,the implant limbic bone resorption was0.47±0.06 mm and 0.71±0.10 mm in the A group,0.41±0.05 mm and 0.58±0.13 mm in the B group.The difference between the two groups was statistically significant at 6-8months and 12-14 months of follow-up.the probing bleeding index was 0.65±0.67 and0.90±0.72 in the A group after 6-8 months and 12-14 months of implant loading,and the probe bleeding index was 0.25±0.44 and 0.45±0.51 in the B group.The difference between the two groups was statistically significant at 6-8 months and 12-14 months of follow-up.There was no significant difference between the two groups at each time point of the modified plaque index.Conclusion: In the scope of this study,the use of internal and external connected implants showed better marginal bone level retention and soft tissue closure in 1 year follow-up compared to implants using platform transfer.
