Effects Of Spleen Strengthening Clearing Damp Combined With HAART On Il-7,ifn-radiation,CD38+ And Hla-dr Immune Cells In HIV/AIDS Patients With Spleen Deficiency And Dampness

Objective:To investigate the effects of spleen strengthening clearing damp combined with HAART on Il-7,ifn-radiation,CD38+ and HLA-DR immune cells in HIV/AIDS patients with spleen deficiency and dampness,and to understand the autoimmune status of HIV patients.The spleen-strengthening clearing damp method combined with HAART was used to treat HIV patients with spleen deficiency and dampness.The expression of related immune indicators in vivo was observed to evaluate the clinical efficacy and safety of spleen-strengthening clearing damp method combined with HAART.Methods: The study sample size of 120 patients,according to the clinical epidemiology experts recommend included 98 patients from July 2017-July 2018,Guang Xi University Of Traditional Chinese Medicine Health Care Center Outpatient Clinic and meet eligibility criteria of the HIV/AIDS patients,with random number table is divided into three groups,Chinese traditional medicine group 24 cases,western medicine group,Chinese and western medicine group 39 cases,35 cases with rui kang affiliated hospital of guangxi medical university physicalexamination center of 22 cases of healthy subjects.the western medicine group was given first-line treatment(AZT/TDF+3TC+EFV/NVP)or second-line treatment(AZT/TDF+3TC+LPV/r),and the choice of the treatment was based on the decision of the infection department physician.Medication: AZT,300 mg,Bid;TDF,300 mg,Qd;3TC,300 mg,Qd;EFV,600 mg,Qn on an empty stomach;NVP,200 mg,first 14 days Qd,second Bid;LPV/r(1 tablet containing LPV 200 mg,RTV 50 mg),2tablets/time,Bid,treatment course was 6 months.In the Chinese and western medicine group,spleen strengthening clearing damp formula(shenling fuzheng capsule 0.37g×4 tablets,Tid)was added on the basis of HAART,and the treatment course was 6 months.In the traditional Chinese medicine group,spleen-strengthening clearing damp(shenling fuzheng capsule 0.37g×4 tablets,Tid)was given,and the treatment course was 6 months.To three treatment group receied six months of observation,respectively in the group before and 6 months after treatment,the determination of IL-7 and numerical interferons alpha cells,CD4 +,CD8 + T cell immune activation molecular CD38,HLA-DR expression,on patients' basic situation,peripheral blood T lymphocyte count,clinical medication safety index as observation index,evaluation method of the spleen clearing damp enhance HAART improve pixu(spleen deficient)wet type the curative effect of people living with HIV.The specific process is as follows:(1)Peripheral venous blood samples were collected before treatment and 6 months after treatment,and il-7 and ifn-beam values were measured before and after treatment in the three groups of patients by elisa,and the effect of spleen strengthening clearing damp method combined with HAART on the expression of CD4+ lymphocyte immune activation markers CD38+ and hla-dr in HIV/AIDS patients was analyzed by BD FACSCalibur flow cytometry.(2)Observe the basic conditions of the patients,and test the safety indicators after 3 and 6 months.SPSS17.0 was used for statistical analysis of patients in each group before and after treatment,and the results were as follows:1,Finally this study included 112 patients,traditional Chinese medicine(TCM)22 cases,western medicine group 32 cases,Chinese and western medicine group 36 cases,health Ti Jian Zu 22 cases,8 cases of loss records,among them,the automatic Chinese traditional medicine group exit 1 case,1 cases of migrant workers,the western medicine group of drug side effects in 2 cases,compliance is poor in 1 case,Chinese and western medicine group automatically exit 1 case,2 cases of drug side effects.2.The results showed that the majority of the patients were middle-aged and elderly,and the majority of the patients were male.3.When comparing the hiv-infected group with the healthy health test group,the expression of il-7 and ifn-pcr in hiv-infected patients was significant(P < 0.05),indicating that the immune response system was chronically activated.4.Comparison of ifn-beam before and after treatment in TCM group,western medicine group and TCM group showed no statistically significant difference between TCM group and western medicine group before treatment(P >,0.05).There was no significant difference between the TCM group and the TCM group(P >,0.05).There was no significant difference between the western medicine group and the western medicine group(61.10±15.26)pg/ml and(61.10±15.26)pg/ml,respectively(P >,0.05).After treatment,pg/ml was 41.10±17.44 in the traditional Chinese medicine group and 38.88±10.62 in the western medicine group,respectively,with no statistically significant difference(P >,0.05).There was significant difference between the TCM group and the TCM group(41.10±17.44)pg/ml and the TCM group(31.92±14.01)pg/ml(P <0.05).The western medicine group and the western medicine group were(38.88±10.62)pg/ml and(38.88±10.62)pg/ml,respectively,with statistically significant differences(P < 0.05).Subjects in the three groups showed a decrease in ifn-beam before and after treatment,and the difference was statistically significant(P < 0.05).5.Before and after treatment,il-7 in the TCM group,western medicine group,and western medicine group was(8.54±3.23)pg/ml,and(9.73±5.44)pg/ml,respectively,before treatment,with no statistically significant difference(P >,0.05).There was no significant difference between the traditional Chinese medicine group and the western Chinese medicine group(8.54±3.23)pg/ml and(9.26±4.87)pg/ml(P >,0.05).There was no significant difference(P >,0.05)between the western medicine group and the western medicine group(9.73±5.44)pg/ml and(9.26±4.87)pg/ml,respectively.After treatment,the TCM group and western medicine group were(6.65±2.07)pg/ml and(7.01±3.33)pg/ml,respectively,with no statistically significant difference(P >,0.05).There was no significant difference between the traditional Chinese medicine group and the western Chinese medicine group(6.65±2.07)pg/ml and5.41±2.61pg/ml(P >,0.05).The western medicine group and the western medicine group were(7.01±3.33)pg/ml and(5.41±2.61)pg/ml,respectively,with statistically significant differences(P < 0.05).There was a statistically significant decrease in il-7 before and after treatment in the three groups(P < 0.05).6.The comparison of CD38 before and after treatment in the TCM group,western medicine group and Chinese and western medicine group showed that before treatment,the CD38 in the TCM group and westernmedicine group were(27.75±12.83)% and(31.52±11.70)%,respectively,with no statistically significant difference(P >,0.05).There was no statistically significant difference between the traditional Chinese medicine group and the western Chinese medicine group(27.75±12.83)% and(29.19±7.04)%(P > 0.05).There was no significant difference between western medicine group(31.52±11.70)% and western medicine group(29.19±7.04)%,respectively(P >,0.05).After treatment,the Chinese medicine group and western medicine group were(13.22±7.31)% and(18.89±6.73)%,respectively,with statistically significant differences(P < 0.05).There was no significant difference between the traditional Chinese medicine group and the western Chinese medicine group(13.22±7.31)%and(12.33±8.47)%,respectively(P >,0.05).Western medicine group and western medicine group were(18.89±6.73)% and(12.33±8.47)%,respectively,the difference was statistically significant(P < 0.05).7.Comparison of hla-dr before and after treatment in the traditional Chinese medicine group,western medicine group and Chinese and western medicine group,before treatment,the traditional Chinese medicine group and western medicine group were(10.12±4.83)% and(9.38±3.22)%,respectively,with no statistically significant difference(P >,0.05).There was no significant difference between the traditionalChinese medicine group and the western Chinese medicine group(10.12±4.83)% and(11.31±5.27)%(P > 0.05).The differences between western medicine group and western medicine group were(9.38±3.22)%and(11.31±5.27)%,respectively,with no statistical significance(P >0.05).After treatment,the Chinese medicine group and western medicine group were(6.59±3.01)% and(7.04±4.49)%,respectively,with statistically significant differences(P >,0.05).There was no significant difference between the traditional Chinese medicine group and the western Chinese medicine group(6.59±3.01%,5.53±2.74%)(P >,0.05).Western medicine group and western medicine group were(7.04±4.49)% and(5.53±2.74)%,respectively,the difference was statistically significant(P < 0.05).8.The number of cases with HIVVL lower than the detection level(50copy/ml)after treatment in the traditional Chinese medicine group,western medicine group and Chinese and western medicine group were 2,30 and 35,respectively,with statistically significant differences(P <0.05).Conclusion:The results show that after treatment between groups of interferons alpha,IL-7,CD38,HLA-DR decreased,CD4 + T cell count increased,HIVRNA group and the Chinese and western medicine group inhibition rate(just below the line)is better,Chinese medicine group is not obvious,prompt the spleen clearing damp method combined HAARTon CD4 + and CD8 + T cell activation,CD38,HLA-DR and interferons alpha,IL-7 expression have cut,inhibition,use method of the spleen clearing damp HIVRNA inhibitory effect is not obvious,but the spleen clearing damp method reduce HAART side effects,clinical application safety.
