Objective:Compared with traditional laparoscopic hysterectomy,the feasibility and safety of single-incision three-channel laparoscopic hysterectomy with conventional instruments through the umbilical approach were explored.The clinical application experience of hysterectomy is summarized in order to provide relevant clinical data for the further promotion and application of this surgical procedure in hysterectomy in the future.Methods:From September 2018 to 2020.3 months,120 patients who chose to be treated in the Obstetrics and Gynecology Department of the Third Clinical Medical College of Inner Mongolia University for Nationalities were required to undergo surgical resection of the entire uterus due to benign diseases.In the hysterectomy group and the traditional laparoscopic total hysterectomy group,there were 60 cases in each group.All enrolled cases were informed in detail before the operation and signed the "Surgery Volunteer".All operations are performed by my tutor.Compare the hemoglobin value and white blood cell count,white blood cell count,operation time,intraoperative blood loss,uterine weight,pain score,postoperative hospital stay,and postoperative 30-day follow-up of the two groups of patients before operation,1 day after operation,and 3 days after operation Whether there are laparoscopic complications such as umbilical hernia,bleeding,infection of incision,etc.,the cosmetic evaluation and satisfaction of this operation are compared and analyzed.Results:59 out of 60 patients in the experimental group successfully completed single-hole laparoscopic total hysterectomy.The other patient had severe pelvic adhesions due to abscesses around the ovary,and the operation was switched to traditional laparoscopic total uterine resection.All.60 patients in the control group successfully completed traditional laparoscopic hysterectomy.Postoperative pathological reports of all patients showed no malignant lesions.The operation time of the patients in the experimental group and the control group were(60.41±11.02)min and(56.50 ±11.84)min;the bleeding volume was(40.51 ± 12.96)ml and(44.58 ± 13.88)ml;the uterine weight was(340.08 ± 159.41)g,(359.47 ± 162.03)g,pain scores were(2.27 ±0.78),(2.57 ± 0.89);postoperative hospital stays were(3.66±0.88)d,(3.82±0.91)d;the above indicators were different There was no statistical significance(P>0.05).According to statistical results,there was no significant difference in hemoglobin value and white blood cell value between the two groups before operation,1 day after operation,and 3 days after operation(P>0.05).The patients were followed up 1 month after surgery,and there were no long-term complications such as umbilical hernia,bleeding,and abnormal vaginal secretions.The satisfaction of the patients in the experimental group and the control group with respect to the degree of beauty of abdominal wounds were(4.46±0.75)points and(4.00±0.84)points;there was a significant statistical difference(P<0.05),and the satisfaction of the experimental group patients was significantly better than the control group.
