| OBJECTIVE This article is to observe whether the application of high-frequency electrical therapy and laser therapy to patients with granulation tissue hyperplasia of airway stenosis will cause differences in the efficacy of patients.Through the comparison of the above two methods,it provides certain clinical experience for the next treatment method selection of patients with airway stenosis caused by the granulation tissue hyperplasia.Methods This article collected patients from the Department of Respiratory Endoscopy of Dalian Central Hospital Affiliated to Dalian Medical University from January 2010 to January 2020 who were treated with high-frequency electrocautery for the treatment of benign airway stenosis with granulation tissue hyperplasia.Another group also collected patients with benign stenosis due to granulation tissue hyperplasia using laser interventional therapy.Obtain case data of two groups of patients,select age,gender,number of bronchoscopy treatment,restenosis time,dyspnea index before and after treatment,postoperative complications and pathological data.The counting data was compared effectively by SPSS23.0 system software.The statistical method adopted rank sum test,and P<0.05 was selected as statistically significant.Result1.A total of 33 patients were included in the group,of which group A(patients with high-frequency electrical therapy for granulation tissue hyperplasia of airway stenosis):15 people.There were 11 males and 4 females.The patients were treated with bronchoscopy for airway stenosis.The total number of treatments was 88,and the average interval of restenosis was 39±21.18 days.There were 2 cases of treatment dueto stenosis caused by bronchial tuberculosis,3 cases of stenosis caused by tracheotomy,and 10 cases of stenosis caused by tracheal intubation.One case involved the left main bronchus,one case involved the middle and lower trachea and the right main bronchus,and 13 cases involved the middle and upper trachea.Group B(patients using laser electrotherapy for granulation tissue hyperplasia of airway stenosis): 18 people.There were 9 male patients and 9 female patients.A total of 133 interventional treatments by bronchoscopy were performed.The average time interval of restenosis was 42.2±41.09 days.Among them,there were 3 cases of interventional treatment for pulmonary tuberculosis,6 cases of stenosis caused by tracheotomy,and 9 cases of stenosis caused by tracheal intubation.All 18 cases involved upper and middle trachea.2.There is no statistically significant difference in the time of restenosis in the treatment of granulation tissue hyperplasia airway stenosis using high frequency electric and laser methods.3.33 patients were enrolled,including 5 patients who had used high frequency treatment for granulation tissue hyperplasia of airway stenosis and laser treatment for granulation tissue hyperplasia of airway stenosis.Analysis of these 5 patients,using Wilcoxon rank sum test,P is 0.225(P<0.05 has statistical significance)shows that the use of high-frequency electricity and laser treatment of granulation tissue hyperplasia of the airway stenosis and then There was no statistical difference in stenosis time.4.The enrolled patients underwent bronchoscopy interventional therapy: 221 times,of which group A(through bronchoscopy high-frequency electrical interventional therapy):a total of 88 treatments,after treatment,80 times were completely effective,accounting for 91%,and some were 9%.Among them,group B(bronchoscope laser interventional treatment)was performed: 133 treatments,after treatment was completely effective:92.4%,partially effective: 6%,mildly effective: 1.5%.Therefore,the effective rate of the enrolled patients after treatment is close to 100%.5.Patients in group A(bronchoscopic high-frequency electrical interventional therapy)used 88 treatments in total,with 80 cases of dyspnea before treatment being grade 4 and8 times of dyspnea before treatment being grade 3.After treatment,50 cases of dyspnea improved to grade 0,and 36 times after treatment,dyspnea improved to grade 1.The grade of dyspnea was significantly improved compared with before.Patients in group B(bronchoscope laser interventional therapy)used a total of 133 treatments,113 dyspnoea before treatment were grade 4 and 20 dyspnea before treatment were grade 3.After treatment,110 cases of dyspnea were improved to grade 0,and 15 times after treatment were improved to grade 1.After treatment,the dyspnea grade was improved to 2 grades.After the patients who entered the group were treated,the patient’s dyspnea grade was significantly better than before treatment.6.After the operation,the patients in group A and group B were observed.After treatment,there were no major bleeding and complications such as perforation.One patient died,and the deceased patient suffered from basic diseases such as hypertension and diabetes during his lifetime,and later died of multiple organ failure.CONCLUSIONS Granulosa tissue hyperplasia patients with benign airway stenosis were treated with high-frequency electrical bronchoscopy and laser bronchoscopy.There was no statistical difference in restenosis time between the two groups.The effective rate of patients after treatment was close to 100%.The dyspnea index decreased significantly.No serious complications such as severe bleeding and tracheal perforation occurred after operation. |
