Construction Of Nursing Sensitive Quality Indicator System For In-patients With Stroke

Objective(1)Based on the evidence-based analysis of the relevant literature on nursing sensitive quality indicators of stroke,the qualitative interview with clinical nursing experts and the Delphi expert consultation,a set of nursing sensitive quality indicators system of stroke was established to meet the actual clinical nursing needs and effectively reflect the quality of nursing servlces。(2)In order to further optimize the indicator system,The established indicator system was initially applied,and the scientificity,applicability and operability of the indicators were comprehensively considered from clinical practice。MethodThis study is a non-experimental study。Firstly,High-quality evidences were obtained through literature review。According to the inclusion criteria and the exclusion criteria,Experts from neurology department who have rich experience in clinical nursing and management of stroke were interviewed to form the first draft of nursing sensitive quality indicator system for in-patients with stroke。To select a certain number of stroke nursing experts and doctors who meet the inclusion criteria in the target hospital for expert interview and pre-investigation,modify and improve the first draft of the indicator system to form the expert consultation draft。After two rounds of expert consultation,Delphi method was used to further modify and improve the indicator system to finally form a clinical test draft。The clinical test draft was applied to clinical practice to evaluate the experimental results in order to further optimize and form the final draft of the indicator system。Result(1)In the process of literature review,1956 database documents were retrieved,and 17 documents were finally included,including 2 IA,1 ⅠB,2 ⅡA,2ⅡB,5 ⅢA,4 ⅢB,and 1 IVA。(2)During the interview,9 clinical stroke experts were interviewed,including 7 nursing experts and 2 doctors。The interview time was 280min and the transcription was about 33000 words。There are two themes:Indicator construction perspective and application vision expectation;six sub-themes:Similar research learning,Based on theoretical model,Patient demand orientation,Grasping construction principles,Focusing on clinical application and Combining quality control tools。(3)The recovery rate of the two rounds of expert consultation questionnaire was 100%and 90.5%respectively,the expert authority coefficient was 0.867 and 0.889 respectively,and the test of coordination coefficient was P<0.001。A nursing sensitive quality indicator system of in-patients with stroke was established with 3 first-class indexes,11 second-class indexes and 14 third-class indexes。The indicator system was applied to clinic,three-month data were collected。Data was collected and analyzed,The indicator system was modified and improved to form the final draft。ConclusionThe nursing sensitive quality indicator system constructed in this study can accurately evaluate the target group,which is scientific、applicable and operable.
