Objectives:The original image data of preoperative CT angiography in neurosurgical patients were reconstructed by three-dimensional image,and the surgical plan wasestablished according to the anatomical relationship between intracranial blood vessels,skull and intracranial lesions in the three-dimensional reconstruction results.Methods:Twenty patients admitted to the department of neurosurgery of the second affiliated hospital of kunling medical university from march,2017 to September,2018 were selected and divided into two groups according to whether they received CTA 3D image reconstruction and post-processing before surgery:3D image reconstruction group(group A)and conventional image group(group B),with 10 patients in each group.All patients in group A underwent CT angiography under 256CT before surgery,and the original image data were imported into the 3D precise surgery planning and analysis software system developed by INCOOL company in DICOM format.After processing by MIP,SSD,VRT,MPR and other technologies,the three-dimensional images of the anatomical relationship between intracranial blood vessels,skull and intracranial lesions were obtained.Then skull,blood vessels,lymph nodes,calcification,tumor in different color display to achieve the best effect,again through the software’s own segmentation tools rotate in any direction,segmentation,by measuring intracranial lesion size,lesion and surrounding skull and vascular anatomical relationship to surgical approach a demo path,choose the best surgical approach.Patients in both groups were treated with microsurgery,and the length of surgery,intraoperative blood loss and length of hospital stay were observed and recorded.The incidence of complications was observed and compared statistically.Results:The three-dimensional reconstruction image clearly showed the three-dimensional relationship between the tumor,the blood supply artery and the skull,with a strong sense of stereoseopic effect.The vital tissues such as the intracranial tumor and blood vessels were stained to accurately restore the intracranial anatomical structure.The three-dimensional image clearly restores the relationship between the tumor body,the size of the tumor neck and the parent artery.Statistical result analysis:compared with the conventional image group,the 3d image reconstruction group had more significant differences in intraoperative blood loss and operation time,which presented statistical significance(p<0.05).The differences in the number of days of hospitalization and the incidence of postoperative complications were not significant(p>0.05).Conclusions:Aceording to the results of three-dimensional reconstruction,the anatomical position relationship between the intracranial lesions and peripheral vessels,skull and other important structures can be observed more intuitively.and the adhesion,compression and wrapping between the tumor and other intracranial lesions and peripheral important vessels can be clearly displayed.Preoperative multi-angle surgical approach can be simulated to achieve the best surgical effect.In the operation,according to the anatomical relationship of the preoperative 3D image,the operation time can be effectively shortened and the intraoperative blood loss can be reduced.so as to improve the success rate and safety of the operation.
