Clinical Study On Thyroid Hormone Changes In Patients With Different Types Of Autoimmune Liver Disease

Objective(s):Cirrhosis of the Liver,in patients with Autoimmune Hepatitis(Autoimmune Hepatitis,AIH)set Free three iodine in the thyroid original glycine(Free Triiodothyronine,FT3)and Free Thyroxine(Free Thyroxine,FT4)after a viral Hepatitis cirrhosis of the Liver and alcoholic Liver cirrhosis are lower,but several different types of Autoimmune Liver Disease,Autoimmune Liver diseases,AILD)in thyroid hormone(Thyroidal Hormones,It is not clear whether there is any difference in the level.Therefore,this test is to understand the changes of TH level in several different types of AILD,and to evaluate whether TH level can assist the classification and diagnosis of AILD.To explore whether TH could evaluate the prognosis of AILD;The correlation between TH and other indicators was analyzed to further explore the correlation between thyroid function and AILD.Methods:1.We collect nearly 2 years AILD cases in the Second People’s Hospital of Yunnan Province,respectively collected AIH 30 patients,29 cases of Primary Biliary Cirrhosis(PBC),1 case of Primary Sclerosing Cholangitis(PSC),19 cases of Overlap Syndrome(OS),collected in the same period with hospitalized patients.A total of 30 patients with normal thyroid function,blood biochemistry,blood coagulation function,and anti-nuclear antibody spectrum and exclusive thyroid and liver disease with upper respiratory tract infection who were hospitalized in the same period were collected as the control group.Thyroid function,blood biochemistry,blood coagulation mechanism and antinuclear antibody spectra were determined,and general baseline data such as gender and age were collected.2.Measurement data with x ± s or the median,comparison with analysis of variance between groups,comparison between count data set by chi-square test,compare the age,sex,body mass index(BMI),prothrombin time(PT),part of the activation of prothrombin time(APTT),triglycerides(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDLC),low density lipoprotein(LDLC)and serum uric acid(UA)and testing for thyroid-stimulating hormone(TSH),FT4 25 indexes such as presence of differences between the groups,analysis and comparison of changes of the biochemical indexes of difference in different groups.3.Compare the difference of TH between the control group and the experimental group(the experimental group was divided into AIH group,PBC group and OS group according to the etiology),discuss the relationship between different types of AILD caused by thyroid hormone,and evaluate whether TH level can assist in the classification and diagnosis of autoimmune liver diseases.4,control group and experimental group(the experimental group was divided into hepatitis and liver cirrhosis group caused by AILD according to the degree of liver injury)were compared weith TH,blood biochemical and the baseline indicators,discusses possible reasons,assess whether thyroid function can be used as evaluation of AILD caused by the severity of liver damage index,discuss whether thyroid function can judge the prognosis of autoimmune liver disease.5.Pearson linear correlation was used to analyze the correlation between thyroid hormone level,baseline level,blood biochemistry and other relevant indicators,to explore the reasons for the correlation,and to further clarify the relationship between thyroid function and AILD.Results:1.The average ages ofA IH group,PBC group and OS group were 57.67 11.58 years,57.86 13.77 years and 53.11 12.66 years,respectively.AIH group,the PBC group,OS compared between two group and the control group,found that OS group than control group,AIH and PBC group of thyroid peroxidase antibodies(TPOAb)concentration is higher,antinuclear antibody spectrum degrees higher,(P<0.05);Pairwise comparison of thyroid hormone levels between the groups showed no significant change(P>0.05).Comparison between groups of PBC group of older,PT,international standardization ratio(INR),APTT,longer TC,direct bilirubin(DBIL)and indirect bilirubin(IBIL),aspertate aminotransferase(AST),alanine aminotransferase(ALT),gamma glutamyl transpeptidase(GGT),higher total protein(TP),albumin(propagated),globulin(GLB)is lower(P<0.01 or P<0.05),and the rest did not see obvious difference.2.The comparison between the hepatitis group and the cirrhosis group indicated that the TH level in the cirrhosis group decreased more significantly,in which FT3 and serum total triiodothyrogen(TT3)were the main components(P<0.05).It was found that the increase of TC,DBIL,IBIL and GGT was more obvious in the cirrhosis group,and the decrease of TP and ALB was more obvious in the cirrhosis group(P<0.05).The elevation of AST and ALT was more obvious in the hepatitis group(P<0.05).3.In the cases collected in this study,TT3 was positively correlated with HDLC,TP and ALB(r=0.323,P=0.001).R=0.414,P=0.000;R=0.474,P=0.000),showing a significant negative correlation with TC and DBIL(r=-0.264,P=0.006;R=0.283,P=0.003);TT4 was positively correlated with TC,HDLC,TP and ALB(r=0.219,P=0.024).R=0.352,P=0.000;R=0.328,P=0.001;R=0.329,P=0.001),TT4 was significantly negatively correlated with TC,DBIL and IBIL(r=-0.249,P=0.009).R=0.253,P=0.008;R=0.204,P=0.034);There was a significant positive correlation between TSH and GLB(r=0.191,P=0.048).There was a significant positive correlation between FT3 and HDLC,TP and ALB(r=0.225,P=0.020).R=0.375,P=0.000;R=0.479,P=0.000;When r=0.578,P=0.000),there was a significant negative correlation between FT3 and TC,DBIL and IBIL(r=-0.275,P=0.004).R=0.273,P=0.004;R=0.252,P=0.008);FT4 was positively correlated with HDLC,TP and ALB(r=0.297,P=0.002).R=0.247,P=0.010;When r=0.342,P=0.000),there was a significant negative correlation between FT4 and TC,DBIL and IBIL(r=-0.219,P=0.023).R=0.219,P=0.023;R=0.198,P=0.040).Conclusion(s):1.Autoimmune liver disease mainly occurs in middle-aged women.2.Thyroid function is not significantly changed in different types of autoimmune liver diseases,which cannot provide help for the diagnosis of different types of autoimmune liver diseases.3.The concentration of TPOAb in OS group was higher than that in AIH group,PBC group and control group,which was considered to be related to the higher concentration of antinuclear antibody spectrum titer and higher in vivo autoimmune activity in OS group.4.Thyroid hormone decreased in the cirrhosis group caused by autoimmune liver disease,especially FT3 and TT3,suggesting that FT3 and TT3 are negatively correlated with the severity of autoimmune liver disease,which can be used as one of the prognostic indicators.5.5.FT3 is positively correlated with HDLC,TP and ALB,suggesting that FT3 was associated with liver synthesis.6.FT3 is negatively correlated with PT,INR,APTT,TBIL,DBIL and IBIL,suggesting a correlation between FT3 and liver metabolism.
