| Purpose:To investigate the value of perfusion imaging with arterial spin labeling(ASL)in patients with vertebral basilar artery distortion.Materials and Methods:1.The vertebral basilar artery distortion patients who met the diagnostic criteria for CTA or MRA in the Affiliated Hospital of Taishan Medical College from December 2015to October 2017 were included.70 of them were enrolled in the experimental group.In the same period,70 patients of similar age and clinical and MRI examinations were selected.Prompt no craniocerebral disease was included in the normal control group.2.The GE Discovery MR 750W 3.0T Superconducting Magnetic Resonance Scanner is used with an 8-channel HEAD-A coil.Before examination with each patient and their families explained that the MRI inspection should pay attention to remove all the metal objects and magnetic cards,check the patient supine position,and scan position to mainta-in immobility to prevent motion artifacts and to appease patients to relax,promptly pin-ch alarms and stop checking if not appropriate.3.Choose from December 2015 to October 2017 70 patients who were diagnosed as vertebral basilar artery distortion by CTA or MRA at the Affiliated Hospital of Taishan Medical College were selected as the observation group and 70 patients with clinical man-ifestations and MRI showed normal controls without craniocerebral disease group.T1WI FLAIR,T2WI PROPELLER,T2WI FLAIR,DWI,3D-TOF MRA,and ASL sequence scans were performed on each patient using a combination of head and neck phaszas.4.Each patient’s scanned images and data are processed and measured at the Functool workstation provided by GE.MRA was imaged by 3D-TOF and reconstructed by three-dimensional MIP(maximum intensity projection).Using GE self-configuration software package,the final ASL perfusion pseudo-color map that cerebral blood flow(relative cerebral blood flow,CBF)pseudo-color map.It has been reported that cerebrovas-cular blood flow in the normal range of the CBF is greater than 50ml/(min·100g),less than50ml/(min·100g)is abnormal,less than 50%of normal is ischemia,there may be a series of ischemia symptom.Using sketching method,the area of 100-200mm2 and the mirror area of ASL-CBF were plotted.The CBF of blood perfusion was recorded 3 times,and the average value was obtained.Finally,statistical analysis was made.Results:Vertebral basilar artery distortion in 70 cases,vertebral artery tortuosity in 38 cases(54.3%),basilar artery tortuosity in 28 cases(40%),vertebrobasilar artery extension and ex-pansion in 28 cases(40%),age(35-60 years),32 cases(45.7%)in the smaller group and38 cases(54.3%)in the larger group(61-83 years old);Two groups of older patients and younger patients with vertebrobasilar arterial distortion were used to perform independent sample t-tests.There was statistical significance between different ages and post-circulator-y CBF values(occipital leaf R/L,P=0.024/0.000;Brain stem R/L,P=0.036/0.028;Cerebell-ar hemisphere R/L,P=0.002/0.032),older patients had lower CBF values in the circulat-ing blood supply area;Vertebrobasilar articulators(experimental group),and the normal CBF value of the vertebral-basal artery(normal group)was statistically significant(Verteb-ral artery falsification,occipital lobe R/L,P=0.002/0.001;brain stem R/L,P=0.000/0.000;cerebellar hemisphere R/L,P=0.000/0.000;basilar artery,occipital lobe R/L,P=0.000/0.000;brain stem R/L,P=0.000/0.000;cerebellar hemisphere R/L,P=0.000/0.000).The CBF value of the experimental group was significantly reduced;Due to the different direc-tion of vertebral artery falsification,bilateral CBF There may be differences in values,but this study does not have statistically significant statistical significance(The right vertebral artery twisted,occipital P=0.454;brain stem P=0.431;cerebellar hemisphere P=0.777;left vertebral artery twisted,occipital P=0.946;brain stem P=0.925;cerebellar hemisphere P=0.006);The CBF values of the vertebral-basal artery dilatation and post-elongation were significantly reduced.Compared with the normal group,there was a statistically significant difference between the bilateral brain stem and cerebellar hemisphere.The bilateral occipi-tal lobe was not statistically significant(occipital leaf R/L,P=0.052/0.085;brain stem R/L,P=0.024/0.016;cerebellar hemisphere R/L,P=0.000/0.000).Conclusion:1.Magnetic resonance 3D-ASL perfusion imaging has a certain diagnostic value for posterior circulation perfusion of vertebral basilar artery distortion.2.In patients with vertebrobasilar arteriolar ectasia,there is a difference in cerebral perfusion after cerebral circulation at different ages.The older,the lower the CBF value in the blood supply area of the posterior circulation of the brain,that is,the more serious the presence of posterior circulation ischemia.3.In the vertebral-basilar artery prolongation and dilatation,the incidence of abnor-mallity in vertebral-basilar arterial falsification is smaller,which may coincide with the falsification.The CBF value in the posterior circulation area of the brain is low,and the degree of posterior circulation ischemia is heavy. |
