| Objective: to investigate the effects of hypertensive disorder complicating pregnancy(HDP)and gestational diabetes mellitus(GDM)on pregnancy outcome in patients with intrahepatic cholestasis of pregnancy(ICP).Methods: a total of 105 patients with ICP delivered in the obstetrics department of the first affiliated Hospital of Xinjiang Medical University from December 2017 to December 2018 were selected as the observation group.According to the presence of HDP and GDM,the observation group was divided into four groups: 70 patients with simple intrahepatic cholestasis of pregnancy,20 patients with intrahepatic cholestasis of pregnancy complicated with hypertensive disorder complicating pregnancy,and 15 patients with intrahepatic cholestasis of pregnancy complicated with gestational diabetes mellitus.210 healthy pregnant women with normal pregnancy and delivery in the same period served as the control group.The clinical data and pregnancy outcome of pregnant women and perinatal infants were collected and statistically analyzed.Results:(1)the incidence of intrauterine distress,premature rupture of membranes,preterm delivery,neonatal asphyxia,stillbirth,amniotic fluid contamination,cesarean section and blood loss in the observation group were significantly higher than those in the control group.(2)the gestational age of ICP+HDP group was earlier than that of ICP group and GDM+ICP group,and the cesarean section rate of ICP+GDM group was higher than that of ICP group,and the difference was statistically significant.The incidence of preterm delivery,intrauterine distress,stillbirth and neonatal asphyxia in ICP+HDP group was higher than that in ICP group,and the birth weight was lower than that in ICP group.The difference was statistically significant.The incidence of preterm delivery,intrauterine distress and neonatal asphyxia in GDM+ICP group was significantly higher than that in HDCP+ICP group.(3)multivariate Logistic regression analysis showed that gestational age ≤ 34 weeks,bile acid ≥ 40 umol / L,HDP and GDM were the risk factors for adversepregnancy outcome in patients with ICP.Conclusion: The incidence of adverse pregnancy outcome of pregnant women with ICP complicated with HDP is high,which requires early detection,timely intervention,strengthening antenatal examination,timely termination of pregnancy and reducing perinatal pregnancy outcome. |
